控制胎头娩出速度助产法减少产妇会阴裂伤的临床观察

摘　　要：目的 探讨控制胎头娩出速度助产法对减少产妇会阴裂伤的效果。方法 选取本院收治的80例经阴道分娩产妇为研究对象,根据随机数字表法分为两组,各40例。对照组采用常规阴道助产保护法,实验组采用控制胎头娩出速度助产法。结果 实验组会阴裂伤发生率为17.50%,低于对照组的45.00%,差异有统计学意义(P<0.05);实验组社会职能、家庭职能、精神状态、休闲时间、食欲水平、日常生活、睡眠效果评分均高于对照组,差异有统计学意义(P<0.05);实验组住院时间短于对照组,产后出血量少于对照组,差异有统计学意义(P<0.05);实验组助产满意度为100%,高于对照组的82.50%,差异有统计学意义(P<0.05)。结论 控制胎头娩出速度助产法应用于经阴道分娩中,能减少产妇会阴裂伤情况发生,缩短住院时间,减少产后出血量,提高生活质量,且产妇助产满意度高,值得临床推广应用。Objective To exploring the effect of controlling the speed of fetal head delivery and assisted delivery on reducing perineal laceration in parturients.Methods 80 postpartum women undergoing vaginal delivery admitted to our hospital were selected as the research subjects,and were divided into two groups according to random number table method,with 40 cases in each group.The control group received routine vaginal midwifery protection,while the experimental group received midwifery by controlling the speed of fetal head delivery.Results The incidence of perineal laceration in the experimental group was 17.50%,lower than 45.00%in the control group,with a statistically significant difference(P<0.05);The scores of social function,family function,mental state,leisure time,appetite level,daily life,and sleep effect in the experimental group were higher than those in the control group,with statistical significance(P<0.05);The hospitalization time of the experimental group was shorter than that of the control group,and the postpartum hemorrhage was less than that of the control group,with a statistically significant difference(P<0.05);The satisfaction rate with midwifery in the experimental group was 100%,higher than 82.50%in the control group,with a statistically significant difference(P<0.05).Conclusion The application of controlled fetal head delivery speed assisted delivery method in vaginal delivery can reduce the occurrence of perineal laceration,shorten hospital stay,reduce postpartum bleeding,improve quality of life,and have high satisfaction with midwifery.It is worth promoting and applying in clinical practice.
