大柴胡汤合大承气汤联合生长抑素治疗轻症急性胰腺炎腑实热结证的临床观察

摘　　要：目的:观察大柴胡汤合大承气汤联合生长抑素治疗轻症急性胰腺炎腑实热结证的临床效果。方法:选取2021年1~10月于我院肝胆外科住院的轻症急性胰腺炎腑实热结证患者60例。采用随机数字表法随机分为试验组和对照组,每组30例。对照组给予生长抑素+西医综合治疗,试验组在对照组治疗基础上增加大柴胡汤合大承气汤经胃管注入治疗,对比两组患者治疗效果。结果:治疗后试验组患者的腹痛缓解时间、腹胀缓解时间和首次排便时间与对照组比较均缩短(P<0.01),中医证候积分低于对照组(P<0.01)。治疗后两组患者白细胞(WBC)、淀粉酶(AMS)、脂肪酶(LPS)水平均较治疗前下降,且试验组下降更明显(P<0.01);治疗后试验组疗效指数优于对照组(P<0.01)。两组患者治疗期间均未发生明显不良反应。结论:在轻症急性胰腺炎腑实热结证患者的治疗上,大柴胡汤合大承气汤联合生长抑素的治疗方案具有更好的临床疗效,可有效降低患者的首次排便时间并缩短腹痛缓解时间、腹胀缓解时间,降低血WBC计数、血AMS、血LPS水平,提高临床疗效。Objective:To observe the clinical effect of dachaihu decoction and dachengqi decoction combined with somatostatin in the treatment of mild acute pancreatitis with syndrome of heat stagnation in fu organ.Methods:60 patients with mild acute pancreatitis with syndrome of heat stagnation in fu organ hospitalized in the hepatobiliary department of our hospital from January to October 2021 were selected,and were randomly divide into the experimental group and the control group by random number table method,with 30 cases in each group.The control group received somatostatin and western medicine comprehensive treatment,and the experimental group received additional treatment with Dachaihu decoction and dachengqi decoction injected through gastric tube on the basis of the control group.The therapeutic effects of the two groups were compared.Results:After treatment,the relief time of abdominal pain,abdominal distension,and first defecation time of patients in the experimental group were shortened compared to the control group(P<0.01),and the traditional Chinese medicine syndrome score was lower than the control group(P<0.01).After treatment,the levels of white blood cells(WBC),amylase(AMS),and lipase(LPS)of patients in both groups decreased compared to those before treatment,and the decrease in the experimental group was more obvious(P<0.01).After treatment,the efficacy index of the experimental group was better than that of the control group(P<0.01).No significant adverse reactions occurred during the treatment period in both groups of patients.Conclusion:In the treatment of patients with mild acute pancreatitis with the syndrome of heat stagnation in fu organ,the combination of dachaihu decoction and dachengqi decoction with somatostatin has better clinical efficacy.It can effectively reduce the first defecation time of patients,shorten the relief time of abdominal pain and abdominal bloating,reduce WBC count,AMS and LPS levels,and improve clinical efficacy.
