股骨颈骨折患者血清IGF-1、BMP-2水平与术后愈合延迟的相关性

摘　　要：目的探讨股骨颈骨折(FNF)患者血清胰岛素生长因子-1(IGF-1)、骨形成蛋白2(BMP-2)水平与患者术后愈合延迟的相关性。方法抽取2019年8月至2021年8月于开封市人民医院接受手术治疗的FNF患者185例作为研究对象,术后随访8个月,采用X线片评估患者术后愈合延迟情况,并将患者分为延迟组与正常组,检测两组患者术后1 d、1周、4周时血清IGF-1、BMP-2水平,分析血清IGF-1、BMP-2水平与FNF患者术后愈合延迟的关系。结果随访8个月时,185例FNF患者中,22例(11.89%,22/185)发生愈合延迟。延迟组术后各时点的血清IGF-1、BMP-2水平均低于正常组(P均<0.05);两组术后血清IGF-1水平均呈上升趋势,但延迟组变化幅度低于正常组;Logistic回归分析结果显示,术后1 d血清IGF-1、BMP-2低表达与FNF患者术后愈合延迟有关(P<0.05)。结论血清IGF-1、BMP-2低表达与FNF患者术后愈合延迟有关,且检测两指标对预测术后愈合延迟的风险具有一定价值。ObjectiveTo investigate the correlation of levels of serum insulin growth factor-1(IGF-1)and bone morphogenetic protein-2(BMP-2)with postoperative delayed healing in patients with femoral neck fractures(FNF).MethodsA total of 185 patients with FNF who received surgery in Kaifeng People’s Hospital from August 2019 to August 2021 were selected as the research subjects.The patients were followed up for 8 months.The postoperative delayed healing was evaluated by X-ray film,and the patients were divided into delayed group and normal group.The levels of serum IGF-1 and BMP-2 in the two group were detected 1 day,1 week and 4 weeks after operation.The correlation of levels of serum IGF-1 and BMP-2 with postoperative delayed healing in FNF patients was analyzed.ResultsAfter 8 months of follow-up,among the 185 FNF patients,22 cases(11.89%,22/185)had delayed healing.Levels of serum IGF-1 and BMP-2 in the delayed group were lower than those in the normal group at each time point after operation(all P<0.05).The serum IGF-1 level in both groups showed an upward trend after operation,but the range of change in the delayed group was lower than that in the normal group.The results of logistic regression analysis showed that the 1-day postoperative low expression of serum IGF-1 and BMP-2 was related to the delayed healing in patients with FNF(P<0.05).ConclusionsThe low expression of serum IGF-1 and BMP-2 is related to the postoperative delayed healing in patients with FNF,and detecting the two indicators has certain value in predicting the risk of postoperative delayed healing.
