摘　　要：目的分析多孔腹腔镜手术、内镜下逆行阑尾炎治疗术在急性阑尾炎中手术效果对比。方法选择2018年1月—2022年1月在本院接受手术治疗急性阑尾炎患者134例为研究对象,以交叉双盲法分为腹腔镜手术组(65例)和内镜手术组(69例)。腹腔镜手术组实施多孔腹腔镜手术,内镜手术组实施内镜下逆行阑尾炎治疗术,观察并对比两组临床指标变化。结果与腹腔镜手术组相比,内镜手术组各项指标均较优,具有统计学差异(P<0.05);内镜手术组术后1 d、术后3 d VAS评分及镇痛药使用率较低,具有统计学差异(P<0.05);内镜手术组手术1 h时后炎症因子水平较低,具有统计学差异(P<0.05);内镜手术组胃肠功能恢复时间、术后进食时间较短,具有统计学差异(P<0.05),内镜手术组总并发症发生率较低(χ^(2)=6.400,P=0.01<0.05)。结论内镜下逆行阑尾炎治疗术治疗急性阑尾炎可促进患者术后胃肠功能恢复,减少术中出血量,降低并发症发生率,取得显著临床效果。Objective To comparethe surgical effects of porous laparoscopic surgery and endoscopic retrograde appendicitis in treating acute appendicitis.Methods 134 patients with acute appendicitisreceived surgical treatment in our hospital from January 2018 to January 2022,as the study objects,were divide into laparoscopic surgery group(65 cases)and endoscopic surgery group(69 cases)according to cross double-blind method.The former underwent porous laparoscopic surgery and the latter received endoscopic retrograde appendicitis treatment.The variation of clinical indicators of the two groups were observed and compared.Results All indexes of the endoscopic surgery group were better than thosein the laparoscopic surgery group,with statistical difference(P<0.05).VAS score and analgesic use rate were lower in the endoscopic surgery group receiving surgeryafter one and three days,with statistical difference(P<0.05).The level of inflammatory factors in the endoscopic surgery group was lower at 1h after operation,with statistical difference(P<0.05).The gastrointestinal function recovery time and postoperative feeding time of the endoscopic surgery group were shorter,with statistical differences(P<0.05),and the total complication rate of the endoscopic surgery group was lower(χ^(2)=6.400,P=0.01<0.05).Conclusions Endoscopic retrograde appendicitis treatment for acute appendicitis can promote the recovery of gastrointestinal function,reduce intraoperative bleeding,reduce the incidence of complications,and achieve significant clinical results.
