小剂量多巴酚丁胺联合呋塞米或托拉塞米对慢性心力衰竭患者的影响

摘　　要：目的:分析小剂量多巴酚丁胺联合呋塞米或托拉塞米对慢性心力衰竭(CHF)患者的影响。方法:选取2019年1月—2022年12月黔南州人民医院收治的100例冠心病CHF患者为研究对象,按照随机数字表法分为观察组和对照组,各50例。两组均采取小剂量多巴酚丁胺治疗,在此基础上观察组采取托拉塞米治疗,对照组采取呋塞米治疗。比较两组患者心功能指标、临床疗效、不良反应发生率。结果:治疗后,观察组左室舒张末期内径、左室收缩末期内径、左室射血分数和血清脑肽钠水平水平优于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P=0.037);观察组不良反应发生率低于对照组,差异有统计学意义(P=0.046)。结论:在CHF患者的治疗中,小剂量多巴酚丁胺与呋塞米或托拉塞米联合均能够改善心功能,但托拉塞米的效果优于呋塞米,用药安全性更高。Objective:To analyze the effects of low-dose dobutamine combined with furosemide or torasemide on patients with chronic heart failure(CHF).Methods:A total of one hundred CHF patients with CHD admitted to Qiannan Prefecture People's Hospital from January 2019 to December 2022 were selected as the study objects,and were divided into observation group and control group according to random number table method,with fifty cases in each group.Both groups were treated with low dose dobutamine,on the basis of which the observation group was treated with torasemide and the control group was treated with furosemide.The cardiac function index,clinical efficacy and incidence of adverse reactions were compared between the two groups.Results:After treatment,left ventricular end-diastolic diameter,left ventricular end-systolic diameter,left ventricular ejection fraction and serum brain peptide sodium level in observation group were better than those in control group,and the differences were statistically significant(P<0.05);The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P=0.037);The incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P=0.046).Conclusion:In the treatment of CHF patients,low-dose dobutamine combined with furosemide or torasemide can improve cardiac function,but the effect of torasemide is better than furosemide,and the drug is safer.
