无创正压通气在ICU重度急性左心衰竭伴Ⅱ型呼吸衰竭抢救中的效果观察

摘　　要：目的:分析无创正压通气在ICU重度急性左心衰竭伴Ⅱ型呼吸衰竭患者抢救中的效果。方法:选取2017年1月—2022年10月铜陵市义安区人民医院收治的ICU重度急性左心衰竭伴Ⅱ型呼吸衰竭患者60例作为观察对象,根据治疗方法不同分为对照组与观察组,各30例。对照组应用鼻部置入导管进行供氧,观察组应用呼吸机经鼻面罩无创正压通气。比较两组患者生命体征、氧动力学指标和血气分析指标、心功能指标。结果:治疗后,观察组患者呼吸频率、舒张压、收缩压、心率低于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者pH值、氧供值、氧耗值、动脉氧分压高于对照组,差异有统计学意义(P<0.05);治疗后,观察组患者Tei指数低于对照组,左室射血分数、每次搏动输出量高于对照组,差异有统计学意义(P<0.05)。结论:应用无创正压通气对ICU重度急性左心衰竭伴Ⅱ型呼吸衰竭患者进行抢救的效果显著,能够有效提高治疗效果,改善患者生命体征、氧动力学指标、血气分析指标、心功能指标。Objective:To analyze the effect of non-invasive positive pressure ventilation in ICU patients with severe acute left heart failure with typeⅡrespiratory failure.Methods:Sixty ICU patients with severe acute left heart failure with typeⅡrespiratory failure admitted to Tongling Yi'an District People's Hospital from January 2017 to October 2022 were selected as the observation objects,and were divided into control group and observation group according to different treatment methods,with thirty cases in each group.The control group was injected with nasal catheter for oxygen supply,and the observation group was injected with non-invasive positive pressure ventilation through nasal mask.The vital signs,oxygen dynamics index,blood gas analysis index and cardiac function index were compared between the two groups.Results:After treatment,the respiratory rate,diastolic blood pressure,systolic blood pressure and heart rate in observation group were lower than those in control group,and the differences were statistically significant(P<0.05);After treatment,the pH value,oxygen supply value,oxygen consumption value and arterial oxygen partial pressure in observation group were higher than those in control group,and the differences were statistically significant(P<0.05);After treatment,Tei index of observation group was lower than control group,left ventricular ejection fraction and output per beat were higher than control group,the difference was statistically significant(P<0.05).Conclusion:The application of non-invasive positive pressure ventilation in ICU patients with severe acute left heart failure with typeⅡrespiratory failure has significant rescue effects,which can effectively improve the treatment effect,and improve the vital signs,oxygen dynamics indexes,blood gas analysis indexes,and cardiac function indexes of patients.
