摘　　要：目的:分析家庭医生签约模式在原发性高尿酸血症患者健康管理中的应用效果。方法:选取2020年1月—2021年12月上海市浦东新区北蔡社区卫生服务中心收治的原发性高尿酸血症患者90例为观察对象,根据随机数字表法分为模型组和签约组,各45例。模型组接受常规护理管理,签约组接受家庭医生签约模式。比较两组患者干预效果。结果:管理后,签约组血尿酸、血肌酐、甘油三酯水平低于模型组,差异有统计学意义(P<0.001);管理后,签约组知信行量表评分高于模型组,差异有统计学意义(P<0.001);管理后,签约组生活质量评分高于模型组,差异有统计学意义(P<0.001)。结论:在原发性高尿酸血症患者的健康管理中应用家庭医生签约模式可以有效地控制其尿酸水平,提高患者的自我管理能力及生活质量。Objective:To analyze the application effect of family doctor contract mode in health management of primary hyperuricemia patients.Methods:A total of ninety patients with primary hyperuricemia admitted to Shanghai Pudong New Area Beicai Community Health Service Center from January 2020 to December 2021 were selected as the observation objects,and were divided into model group and contract group according to random number table method,with forty-five cases in each group.The model group accepted routine nursing management,and the contract group accepted the contract mode of family doctors.The Intervention effect of the two groups was compared.Results:After management,the levels of serum uric acid,serum creatinine and triglyceride in the contract group were lower than those in the model group,with statistical significance(P<0.001);After management,the score of the contract group was higher than that of the model group,the difference was statistically significant(P<0.001);After management,the life quality score of the contract group was higher than that of the model group,and the difference was statistically significant(P<0.001).Conclusion:The application of family doctor contract model in the health management of patients with primary hyperuricemia can effectively control their uric acid level,improve their self-management ability and quality of life.
