摘　　要：目的探讨心脏磁共振评价心肌梗死经皮冠状动脉介入术(PCI)治疗的效果。方法对2018年10月至2019年10月浙江绿城心血管病医院进行PCI治疗的心肌梗死25例患者治疗情况以及相关资料进行回顾分析,计算并比较手术前后心脏磁共振成像(CMR)钆对比剂延迟强化(LGE)的左心室透壁程度检测结果中存活心肌以及非存活心肌个数。结果手术前CMR-LGE检测结果中异常心肌节段共计168个,存活心肌共计93个,非存活心肌共计75个,PCI后原CMR-LGE检测结果中的168个异常心肌节段情况与手术前相比有所变化,存活心肌共计107个,非存活心肌共计61个。与正常心肌组织区域首过灌注时间对比来看,病变部位心肌组织的首过灌注时间更长(t=17.798,P<0.01)。结论通过CMR检查能够准确了解心肌梗死患者心肌组织的存活程度和存活位置,有利于临床治疗方案的准确制定。Objective To explore the evaluation efficacy of cardiac magnetic resonance(CMR)of percutaneous coronary intervention(PCI)for myocardial infarction.Methods A retrospective analysis was conducted on the treatment and relevant data of 25 patients with myocardial infarction who underwent PCI surgery in Zhejiang Greentown Cardiovascular Hospital from October 2018 to October 2019.The number of viable myocardium and nonviable myocardium in the results of left ventricular(LV)transmural extent using CMR-late gadolinium enhancement(LGE)before and after surgery was calculated and compared.Results In total,there were 168 abnormal myocardial segments in the CMR-LGE detection results before surgery,including 93 viable myocardia and 75 non-viable myocardia.After PCI surgery,there were changes in the original 168 abnormal myocardial segments detected by the CMR-LGE results before surgery,with 107 viable myocardia and 61 non-viable myocardia.Compared with the normal myocardial tissue area,the first-pass perfusion time of the myocardial tissue in the lesion site was longer(t=17.798,P<0.01).Conclusion Through CMR examination,it is possible to get an accurate picture of the survival extent and survival location of myocardial tissue in patients with myocardial infarction,which is beneficial for the accurate formulation of clinical treatment plans.
