摘  要:目的 分析连续动态随访护理模式在老年高血压中的干预效果。方法 选取2021年3月至2023年3月厦门市翔安区新店中心卫生院收治老年高血压患者,随机分为研究组和对照组。对照组实施常规随访护理,研究组实施连续动态随访护理,比较两组护理前后疾病认知水平、自我效能感、自我护理能力、血压水平及健康素养水平。结果 共纳入92例高血压患者,两组各46例。护理后,研究组疾病认知水平、自我效能感、自我护理能力评分高于对照组,舒张压、收缩压水平低于对照组,健康素养评分高于对照组,差异均有统计学意义(均P<0.05)。结论 采用连续动态随访护理模式可加深老年患者对高血压的认识,增强自我效能感,进而提高自我护理能力,降低血压水平,维持良好的健康状态。Objective To analyze the intervention effect of continuous dynamic follow-up nursing model in elderly hypertension.Methods Elderly hypertensive patients admitted to Xindian Central Health Center,Xiang'an District,Xiamen City,from March 2021 to March 2023 were selected and randomly divided into the study group and control group.The control group implemented routine follow-up care,and the study group implemented continuous dynamic follow-up care,comparing the level of disease cognition,self-efficacy,self-care ability,blood pressure level,and health literacy level before and after care in the two groups.Results A total of 92 hypertensive patients were included,46 in each of the two groups.After nursing,the study group's disease cognition level,self-efficacy and self-care ability scores were higher than those of the control group,diastolic and systolic blood pressure levels were lower than those of the control group,and health literacy scores were higher than those of the control group,and the differences were all statistically significant(all P<0.05).Conclusions The use of continuous dynamic follow-up nursing model can deepen the elderly patients'understanding of hypertension,enhance the sense of self-efficacy,and then improve the self-care ability,reduce the level of blood pressure,and maintain a good state of health.
