基于螺旋CT测量肾周脂肪厚度预测供肾者分肾功能的价值

摘　　要：目的本研究的目的是探讨应用螺旋CT测得的肾周脂肪厚度在评估分肾功能中的临床价值。方法回顾性分析我院173例活体供肾者的AVU、肾动态显像及临床资料,在整个研究人群中,我们将肾周脂肪厚度(perirenal fat thickness,PrFT)分为P rFT-L侧、P rFT-L后、P rFT-R侧、P r FT-R后,分别研究与其他肥胖参数、代谢危险因素及99mTc-DTPA(99mTechnetium diethylenetriamine pentaacetic acid)肾动态显像测得的肾小球滤过率(Gates-GFR)的相关性,分析肾周脂肪厚度在评估分肾功能中的临床价值。结果173例活体供肾者中男性60名,女性113名,平均年龄54.75±7.22岁,PrFT-L侧平均为15.94±6.14mm,与左肾GatesGFR呈负相关(r=-0.515,P<0.001),PrFT-R侧平均约12.16±6.16 mm,与右肾Gates-GFR呈负相关(r=-0.413,P<0.001)。结论我们的结果证实了肾周脂肪厚度与分肾肾小球滤过率(Gates-GFR)具有良好的相关性,肾周脂肪可能构成肾小球滤过率降低的潜在危险因素。Objective the purpose of this study is to investigate the application of spiral CT of renal fat thickness in evaluating clinical value of renal function.Methods retrospective analysis of 173 cases for the kidney AVU,renal dynamic imaging and clinical data,in the whole group,we will renal week Fat Thickness(Perirenal Fat Thickness,PrFT)is divided into PrFT-side,after PrFT-L,PrFT-R side,PrFT-R,respectively,research and other parameters,metabolic risk factors,and 99 mTC-DTPA(99mTechnetium diethylenetriamine pentaacetic acid)dynamic imaging of renal glomerular filtration rate(Gates-GFR)correlation,analysis of renal week Fat Thickness in evaluating the clinical value of renal function.Results 173 cases of living donor kidney 60 of the men and women in 113,the average age of 54.75±7.22 years,PrFT-L side with an average of 15.94±6.14mm,and negatively correlated with left renal Gates-GFR(r=0.515,P<0.001),PrFT-R side an average of about 12.16±6.16 mm,and negatively correlated to the right kidney Gates-GFR(r=0.413,P<0.001).Conclusion our results confirmed the week of kidney fat thickness and points of renal glomerular filtration rate(Gates-GFR)has good correlation,renal week fat may constitute the glomerular filtration rate to reduce the potential risk factors.
