输尿管结石患者应用术前全体积CT纹理联合输尿管软镜钬激光治疗的效果评价

摘　　要：目的观察术前全体积CT纹理评价联合输尿管软镜钬激光对上段输尿管结石的治疗效果。方法回顾性选取2016年8月-2021年8月在我院接受治疗的上段输尿管结石患者为研究对象,根据其术前是否进行全体积CT纹理评价分为对照组(60例)和观察组(60例)。观察两组手术一般情况,比较两组治疗前后肾功能、炎症因子和并发症发生率的差异。结果观察组恢复得更快,结石清除率较对照组高(P<0.05);两组手术前炎症因子水平无差别,术后1周,观察组炎症因子水平低于对照组(P<0.05);两组术后1月和术后3月IPSS和Qmax水平比较,差异无统计学意义;两组结石上移残留、排尿困难和肾周积液发生率比较,差异无统计学意义(P=0.675)。结论术前全体积CT纹理评价联合输尿管软镜钬激光对上段输尿管结石的治疗效果较好,因其对结石的性状有很好的分析,可缩短术中所需时间,减轻术后应激,具有良好的应用价值。Objective To observe the effect of preoperative full-volume CT texture evaluation combined with flexible ureteroscope and holmium laser on upper ureteral calculi.Methods Patients with upper ureteral calculi who received treatment in our hospital from August 2016 to August 2021 were retrospectively selected as the study subjects,and were divided into control group(60 cases)and observation group(60 cases)according to whether they had undergone preoperative volume CT texture evaluation.The general operation conditions of the two groups were observed,and the differences in renal function,inflammatory factors and complication rates between the two groups before and after treatment were compared.Results The observation group recovered faster and had a higher stone clearance rate than the control group(P<0.05);there was no difference in the levels of inflammatory factors before operation between the two groups.One week after the operation,One week after the procedure,the observed inflammatory factor levels were lower than in the control group(P<0.05);there was no statistically significant difference in the levels of IPSS and Qmax between the two groups at 1 month and 3 months after surgery;There was no statistically significant difference in the incidence of residual stones,dysuria,and perinephric effusion(P=0.675).Conclusion Preoperative total volume CT texture evaluation combined with soft ureteroscope Holmium laser has a good therapeutic effect on upper ureteral calculi,which can shorten the operative time and reduce postoperative stress due to its good analysis of the characteristics of calculi.Therefore,it has good application value.
