摘 要:目的探讨不同类型附件肿瘤的评价(ADNEX)模型融入卵巢-附件报告和数据系统(O-RADS)超声风险分层在良恶性卵巢肿块鉴别诊断中的应用价值。方法回顾性分析311例经超声检查发现卵巢肿块并接受手术治疗的患者的临床资料及超声图像。分别使用ADNEX模型、O-RADS超声风险分层、ADNEX模型融入O-RADS超声风险分层评估卵巢肿块的性质。以病理结果为金标准,绘制受试者工作特征(ROC)曲线,评价3种方法对良恶性卵巢肿块的鉴别诊断价值。结果311例患者共计340个肿块,病理结果证实良性肿块263个、恶性肿块77个。ROC曲线分析显示,ADNEX模型、O-RADS超声风险分层、ADNEX模型融入O-RADS超声风险分层鉴别诊断良恶性卵巢肿块的曲线下面积分别为0.899、0.861、0.904,三者比较差异无统计学意义(P>0.05),但ADNEX模型融入O-RADS超声风险分层的特异度及阳性预测值高于另外两种方法(P<0.05)。结论ADNEX模型融入O-RADS超声风险分层对良恶性卵巢肿块具有良好的鉴别诊断效能。Objective To explore the application value of Assessment of Different Neoplasia in the Adnexa(ADNEX)model integrated into Ovarian-Adnexal Reporting and Data System(O-RADS)ultrasonic risk stratification in the differential diagnosis between benign and malignant ovarian masses.Methods The clinical data and ultrasonic image of 311 patients with ovarian masses detected by ultrasound and treated by surgery were retrospectively analyzed.The ADNEX model,O-RADS ultrasonic risk stratification,ADNEX model integrated into O-RADS ultrasonic risk stratification were used to evaluate the nature of ovarian masses,respectively.With pathological results as the gold standard,the receiver operating characteristic(ROC)curve was drawn to evaluate the value of three methods in the differential diagnosis on benign and malignant ovarian masses.Results There were totally 340 masses in 311 patients,and the pathological results verified 263 benign masses and 77 malignant masses.The ROC curve analysis indicated that areas under the curve of ADNEX model,O-RADS ultrasonic risk stratification,and ADNEX model integrated into O-RADS ultrasonic risk stratification for differentially diagnosing benign and malignant ovarian masses were 0.899,0.861,and 0.904,respectively,no statistically significant difference was found between the 3 methods(P>0.05),but the specificity and positive predictive value of ADNEX model integrated into O-RADS ultrasonic risk stratification were higher than those of the other two methods(P<0.05).Conclusion ADNEX model integrated into O-RADS ultrasonic risk stratification exerts favorable efficiency of differential diagnosis on benign and malignant ovarian masses.
