摘 要:目的探讨肺内病变患者应用同轴活检针进行活检的效果和安全性。方法回顾性分析2020年5月至2022年10月保定市人民医院收治的115例肺内病变患者资料,按照穿刺方法不同分组。对照组(53例)采用非同轴活检针穿刺活检,试验组(62例)采用同轴活检针穿刺活检。比较两组手术时间、取材成功率及并发症发生率。结果试验组平均手术时间短于对照组,差异有统计学意义(P<0.05)。两种技术的取材成功率均较高,差异无统计学意义(P>0.05)。试验组气胸发生率低于对照组,差异有统计学意义(P<0.05)。亚组分析中,当病变长径<2.5 cm时,试验组气胸发生率低于对照组;当穿刺深度≥3 cm时,试验组出血、气胸发生率均低于对照组,差异有统计学意义(P<0.05)。结论应用同轴技术可缩短肺内病变穿刺活检的手术时间,降低气胸发生率,尤其当病变长径<2.5 cm、穿刺深度≥3 cm时,建议使用同轴活检针进行穿刺活检,以减少并发症发生。Objective The effectiveness and safety of using coaxial biopsy needles for biopsy in patients with pulmonary lesions were explored.Methods With the retrospective analysis of the data of 115 patients with pulmonary lesions admitted to Baoding People's Hospital from May 2020 to October 2022,patients with non-coaxial biopsy were divided into the control group(53 cases),while patients with coaxial biopsy were divided into the coaxial group(62 cases),according to different puncture methods.The surgical time,success rate of material collection,and incidence of complications were compared between the two groups.Results The average surgical time in the coaxial group was shorter than that in the control group,and the difference was statistically significant(P<0.05).The success rates of both techniques were relatively high,and the difference was not statistically significant(P>0.05).The incidence of pneumothorax in the coaxial group was lower than that in the control group,and the difference was statistically significant(P<0.05).In subgroup analysis,when the lesion length diameter was less than 2.5 cm,the incidence of pneumothorax in the coaxial group was lower than that in the control group;When the puncture depth was≥3 cm,the incidence of bleeding and pneumothorax in the coaxial group was lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of coaxial technology can shorten the surgical time for needle biopsy of pulmonary lesions and reduce the incidence of pneumothorax.In addition,especially when the length of the lesion is less than 2.5 cm and the puncture depth is≥3 cm,it is recommended to use coaxial technique for puncture biopsy,which can reduce the occurrence of complications.
