摘 要:目的根据XN1000-B4全自动血液分析仪中“PLT Clumps”的Q-Flag报警值判断低值血小板(PLT)结果的可靠性。方法通过对XN1000-B4全自动血液分析仪分析的低值PLT标本进行人工推片镜检,积累标本的Q-Flag报警值的分布及对应的PLT聚集情况,进行灵敏度和特异度分析并绘制ROC曲线。结果200份PLT聚集阳性标本的Q-Flag报警值分布于20~300之间。收集400份Q-Flag报警值>20的低值PLT标本进行人工推片镜检,结果显示,不同Q-Flag报警值分组的PLT聚集情况(阴性、阳性)比较,差异有统计学意义(χ^(2)=97.547,P=0.000);且随着Q-Flag报警值的增高,PLT聚集的阳性率增高,趋势性χ^(2)检验显示差异有统计学意义(趋势性χ^(2)=0.494,P=0.000);当Q-Flag报警值≥250时,PLT聚集阳性的灵敏度为95.0%。收集Q-Flag报警值<30的200份低值PLT标本进行人工推片镜检,结果显示,Q-Flag报警值<20时标本未发生PLT聚集的特异度为100.0%。ROC曲线提示,以Q-Flag报警值预测PLT聚集情况的最佳临界值为100,此时曲线下面积为0.900,灵敏度为86.5%,特异度为75.6%。结论通过XN1000-B4全自动血液分析仪“PLT Clumps”的Q-Flag报警值判断低值PLT标本是否发生PLT聚集具有较高的可行性,其应用不仅利于检验工作者提高工作效率,而且更好地保证了检验质量。Objective The reliability of low value platelet(PLT)results was determined based on the Q-Flag alarm value of"PLT Clumps"in the XN1000-B4 automatic blood analyzer.Methods Through manual slide microscopic examination of low value PLT samples analyzed by XN1000-B4 automatic hematology analyzer,the distribution of Q-Flag alarm values and corresponding PLT aggregation of samples were accumulated.In addition,sensitivity and specificity analysis was analyzed,and the ROC curve was drawn.Results The Q-Flag alarm values of 200 PLT aggregation positive specimens ranged from 20 to 300.With collection of 400 low value PLT samples with Q-Flag alarm values greater than 20 for manual slide microscopy,the results showed that there was a statistically significant difference in PLT aggregation(negative and positive)between different Q-Flag alarm value groups(χ^(2)=97.547,P=0.000);In addition,the positive rate of PLT aggregation increased with the increase of Q-Flag alarm value,and the trendχ^(2)test showed a statistically significant difference(trendχ^(2)=0.494,P=0.000);When the Q-Flag alarm value was≥250,the sensitivity of PLT aggregation positive was 95.0%.With collection of 200 low-value PLT specimens with Q-Flag alarm value<30 for artificial microscope examination,the results showed that the specificity of specimens without PLT aggregation with Q-Flag alarm values<20 was 100.0%.The ROC curve indicated that the optimal critical value for predicting PLT aggregation with the Q-Flag alarm value was 100.At this time,the area under the curve was 0.900,the sensitivity was 86.5%,and the specificity was 75.6%.Conclusions The Q-Flag alarm value of XN1000-B4 automatic blood analyzer"PLT Clumps"has a high feasibility in determining whether PLT aggregation occur in low-value PLT specimens.Its application is not only conducive to improving the work efficiency of laboratory workers,but also better ensures the quality of examination.
