摘　　要：目的:比较人工关节置换与内固定治疗骨质疏松性不稳定股骨粗隆间骨折的效果。方法:研究样本选取2020年1月至2022年6月期间于台山市人民医院就诊的80例骨质疏松性不稳定股骨粗隆间骨折患者,随机均分为对照组与观察组。对照组患者实施人工关节置换术,观察组患者实施内固定手术,对比两组患者的恢复情况、炎症指标、术后并发症。结果:在术前,两组患者的Harris评分、骨密度相比,P>0.05。在术后1个月、术后3个月,观察组患者的Harris评分与骨密度均低于对照组,P<0.05。在术后6个月、术后12个月,观察组患者的Harris评分与骨密度均高于对照组,P<0.05。在术前,两组患者的IL-6、IL-8、TNF-α、CRP水平、VAS评分相比,P>0.05。在术后2周,观察组患者的IL-6、IL-8、TNF-α、CRP水平、VAS评分均低于对照组,P<0.05。观察组的术后并发症发生率低于对照组,P<0.05。结论:相较于人工关节置换,骨质疏松性不稳定股骨粗隆间骨折实施内固定治疗的创伤性较小,患者术后并发症较少,更易获得良好的恢复效果。Objective:To compare the efficacy of artificial joint replacement and internal fixation in the treatment of osteoporotic unstable intertrochanteric fracture of femur.Methods:A total of 80 patients with osteoporotic unstable intertrochanteric fracture of femur treated in Taishan People’s Hospital from January 2020 to June 2022 were selected and randomly divided into control group and observation group.The control group underwent artificial joint replacement and the observation group underwent internal fixation.The recovery,inflammation indexes and postoperative complications of the two groups were compared.Results:Before surgery,the Harris score and bone mineral density of the two groups were compared,P>0.05.At 1 month and 3 months after surgery,the Harris score and bone mineral density of observation group were lower than those of control group(P<0.05).At 6 months and 12 months after surgery,the Harris score and bone mineral density of observation group were higher than those of control group(P<0.05).Before surgery,the levels of IL-6,IL-8,TNF-α,CRP and VAS scores in 2 groups were compared,P>0.05.At 2 weeks after operation,the levels of IL-6,IL-8,TNF-α,CRP and VAS in observation group were lower than those in control group(P<0.05).The incidence of postoperative complications in the observation group was lower than that in the control group,P<0.05.Conclusion:Compared with artificial joint replacement,the treatment of osteoporotic unstable femoral intertrochanteric fracture with internal fixation is less traumatic,less postoperative complications,and more likely to obtain a good r ecovery effect.
