半失能老年人心理健康、照护依赖情况调查及与主要照顾者健康权利赋能的关系

摘　　要：目的:调查半失能老年人心理健康和照护依赖情况,并分析半失能老年人心理健康、照护依赖与主要照顾者健康权利赋能之间的关系。方法:回顾性分析2021年10月-2022年10月抽样调查的120例半失能老年人的基本资料及其主要照顾者健康权利赋能情况。统计半失能老年人性别、年龄、失能状态、心理健康、照护依赖分布情况。根据半失能老年人心理健康情况将其分为心理健康组(老年人心理健康量表评分≥30分,n=51)和心理异常组(老年人心理健康量表评分<30分,n=69),比较两组半失能老年人主要照顾者性别、年龄、文化程度、月收入、日照护时间、照护年限等健康权利赋能情况。再根据半失能老年人照护依赖情况[中文版照护依赖量表(CDS-R)]将其分为轻度依赖组(CDS-R≤30分,n=33)、中度依赖组(30分<CDS-R≤60分,n=50)、重度依赖组(CDS-R>60分,n=37),比较3组半失能老年人主要照顾者性别、年龄、文化程度、月收入、日照护时间、照护年限等健康权利赋能情况,分析半失能老年人心理健康、照护依赖情况与主要照顾者健康权利赋能的关系。结果:120例半失能老年人中以男性、高龄老年人、轻度失能者、心理异常者、中度依赖者更为多见。心理健康组与心理异常组两组主要照顾者文化程度、月收入、日照护时间、照护年限比较差异有统计学意义(χ^(2)=6.774,14.287,12.787,19.303;P<0.05)。轻度依赖组、中度依赖组、重度依赖组3组主要照顾者日照护时间、照护年限比较差异有统计学意义(χ^(2)=31.286,12.744;P<0.05)。主要照顾者月收入、日照护时间、照护年限均是影响半失能老年人心理健康的影响因素(OR=3.758,3.861,4.367;P<0.05)。主要照顾者日照护时间、照护年限是影响半失能老年人照护依赖情况的影响因素(OR=4.509,4.730;P<0.05)。结论:半失能老年人多为心理异常者和照护依赖者,其主要照顾Objective:To investigate the psychological health and care dependence status of the semi-disabled elderly,and to analyze the relationship between psychological health,care dependence and the health right empowerment of primary caregivers.Methods:Clinical data of 120 semi-disabled elderly people who were sampled in the hospital from October2021 to October 2022 and health right empowerment of their primary caregivers were retrospectively analyzed.The distribution status of gender,age,disability status,psychological health and care dependence of semi-disabled elderly people were analyzed.According to the psychological health status ofsemi-disabled elderly people,they were divided into healthy psychology group(elderly psychological health scale score≥30 points,n=51)and abnormal psychology group(elderly psychological health scale score<30 points,n=69).The gender,age,education level,monthly income,daily care time,care years and other health right empowerment status of the primary caregivers of the two groups of semi-disabled elderly people were compared.By means of care dependence of semi-disabled elderly people[Chinese version of Care Dependence Scale(CDS-R)],they were classified intomild dependence group(CDS-R≤30 points,n=33),moderate dependence group(30 points<CDS-R≤60 points,n=50)and severe dependence group(CDS-R>60 points,n=37).The three groups were compared with the gender,age,education level,monthly income,daily care time,care years and other health right empowerment of the primary caregiver of semi-disabled elderly people,and the relationship between psychological health and care dependence of semi-disabled elderly people and health right empowerment of the primary caregivers was analyzed.Results:A total of 120 semi-disabled elderly people were mainly males,senior elderly,mild disabled people,people with psychological abnormalities and people with moderate dependence.There were statistically significant differences in the educational level,monthly income,daily care time and care years of the primary careg
