摘 要:目的分析近年来我院就诊的妊娠期女性TORCH感染情况,为本地区优生优育工作提供参考。方法选取2016年至2020年来我院就诊的4014例妊娠期女性为研究对象。采集患者血清,利用酶联免疫反应实验检测血清中TORCH各病原体抗体IgG和IgM的阳性情况,并对数据进行处理和分析。结果妊娠期女性的TORCH检测率随孕周增加而增加。TOX、RV和HSV-1的感染在不同年龄的妊娠女性中存在差异。各孕期IgG的阳性率由高到低均为CMV、RV、HSV-1、HSV-2、TOX,孕早期和孕中期IgM的阳性率由高到低为HSV-1、HSV-2、CMV、RV、TOX,孕晚期RV IgM阳性率高于CMV。TORCH的合并感染以双重感染居多,其中IgG阳性的合并感染以RV、CMV、HSV-1为主,IgM阳性的合并感染以HSV-1和HSV-2为主。相较于RV、CMV和HSV-1,妊娠期女性对TOX和HSV-2的整体免疫基础较弱,且4种感染模式在不同孕期存在较显著的差异(P<0.001)。妊娠女性的整体和各孕期的感染模式分析中,IgG+/IgM+感染模式HSV-1阳性率最高,IgG-/IgM+感染模式HSV-2阳性率最高。本地区季节变换对妊娠期女性TORCH的感染无影响。结论本地区妊娠期女性应着重注意对HSV感染的预防,避免复发和再次感染。Objective To analyze TORCH infection in pregnant women of our hospital in recent years,and to provide a knowledge reference for the practice of prenatal and postnatal care in our region.Methods We selected 4014 pregnant women who visited our hospital during 2016-2020 as the study objects.The maternal serum was collected and used for detecting the TORCH pathogen antibody IgG and IgM using enzyme-linked immunoreaction test,and the data were processed and analyzed.Results TORCH detection rate of pregnant women increased with gestational age.The positive rates of IgG during pregnancy were ranked as:CMV,RV,HSV-1,HSV-2,and TOX,while the positive rates of IgM in the first and second trimesters were ranked as:HSV-1,HSV-2,CMV,RV,and TOX,and the IgM positive rate of RV was more than CMV in the third trimester.The main category of TORCH co-infections was dual-infection,and the IgG-positive pathogens of co-infection were mainly RV,CMV,and HSV-1.The primary IgM-positive pathogens of co-infection were HSV-1 and HSV-2.Compared with RV,CMV,and HSV-1,the pregnant women’s immunocompetence of TOX and HSV-2 was weaker,and the infection pattern of TOX and HSV-2 showed discrepancies among different pregnancies(P<0.001).The whole and individual pregnancy infection pattern analysis showed that HSV-1 and HSV-2 had the highest positive rate in IgG+/IgM+and IgG-/IgM+infection patterns,respectively.Season had no effect on pregnant women TORCH infection in local.Conclusion Pregnant women should pay attention for the prevention of HSV infection and to avoid recurrence and reinfection.
