摘 要:目的:探讨高血压前期患者注意功能损伤相关脑神经网络的变化情况及其脑功能异常的作用机制。方法:收集2018年3月—2022年12月间在汕头大学医学院第一附属医院就诊的12例高血压前期患者为高血压前期组,同期招募的12名健康志愿者为正常对照组。采用蒙特利尔认知评估量表(Montreal Cognitive Assessment,MoCA)和医院焦虑抑郁量表评估研究对象的认知功能及精神状态,采用空间Stroop-Simon任务范式测试研究对象的注意功能,采用脑功能磁共振成像(functional magnetic resonance imaging,fMRI)研究注意功能相关脑区的激活状况。结果:高血压前期组MoCA总分低于对照组(P<0.05)。空间Stroop-Simon任务行为学测试结果显示,与对照组相比较,高血压前期组的反应速度及正确率差异无统计学意义(P>0.05)。脑fMRI显示高血压前期组在双侧楔前叶、前额叶和前扣带回皮层脑区的激活相比对照组增强(P<0.05),高血压前期组没有激活低于正常组的脑区。结论:高血压疾病相关的认知功能损伤可能发生在高血压前期,高血压前期患者的注意功能相关脑区出现了异常的激活增强。Objective:To investigate the changes in brain neural networks associated with attentional impairment in prehypertensive patients and the mechanism of action of their abnormal brain function.Methods:Twelve prehypertension patients attending the First Affiliated Hospital of Shantou University Medical College between March 2018 and December 2022 were collected as the prehypertension group,and 12 healthy volunteers recruited during the same period as the normal control group.The Montreal Cognitive Assessment(MoCA)and Hospital Anxiety and Depression Scale were used to assess the cognitive function and mental state of the study subjects,the Spatial Stroop-Simon Task Paradigm was used to test the attentional function of the study subjects,and functional magnetic resonance imaging(fMRI)was used to study the activation status of brain regions related to attention function.Results:The total MoCA score of the prehypertension group was lower than that of the control group(P<0.05).The results of spatial Stroop-Simon task-behavioral test showed no statistically significant difference in response speed and correctness in the prehypertension group compared with the control group(P>0.05).Brain fMRI showed enhanced activation in bilateral precuneus,prefrontal,and anterior cingulate cortex brain regions in the prehypertension group compared with the control group(P<0.05),and there was no activation in the prehypertension group that was lower than that in the normal group.Conclusion:Cognitive impairment associated with hypertensive disorders may occur in prehypertension,and patients with prehypertension show abnormal enhancement of activation in brain regions related to attentional function.
