金蝉止痒胶囊联合左西替利嗪治疗慢性荨麻疹的疗效

摘　　要：目的:观察金蝉止痒胶囊联合左西替利嗪治疗慢性荨麻疹(CU)的疗效。方法:选取86例CU患者分为单纯西药组(左西替利嗪)和中西联合组(金蝉止痒胶囊+左西替利嗪),每组各43例,两组均治疗1个月。比较两组疗效,治疗前后临床症状积分,荨麻疹活动性评分(UAS)和皮肤病生活质量指数评分(DLQI),血清补体(C3、C4),血清炎症因子[白细胞介素4(IL-4)和肿瘤坏死因子ɑ(TNF-ɑ)]及免疫球蛋白E(IgE)及两组不良反应发生情况。结果:治疗后,中西联合组总有效率(95.35%)高于单纯西药组(81.40%)(P<0.05);治疗后,两组临床症状积分(风团数目、大小及持续时间和瘙痒症状)均降低(P<0.05),且中西联合组低于单纯西药组(P<0.05);治疗后,两组UAS和DLQI评分均降低(P<0.05),且中西联合组低于单纯西药组(P<0.05);治疗后,两组血清C3水平均升高(P<0.05),且中西联合组高于单纯西药组(P<0.05);治疗后,两组IL-4、IgE及TNF-ɑ水平均降低(P<0.05),且中西联合组低于单纯西药组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:金蝉止痒胶囊联合左西替利嗪治疗CU患者疗效确切,能有效缓解患者症状,升高血清补体,降低炎症因子水平。Objective:To observe the efficacy of Jinchan Zhiyang capsules combined with levocetirizine in the treatment of chronic urticaria(CU).Methods:86 patients with CU were selected and divided into 43 cases in simple western medicine group(levocetirizine)and 43 cases in Chinese-western medicine group(Jinchan Zhiyang capsules+levocetirizine).Both groups were treated for 1 month.The efficacy,and clinical symptoms scores,Urticaria Activity Score(UAS),Dermatological Life Quality Index(DLQI),serum complements(C3 and C4),serum inflammatory factors[interleukin-4(IL-4),tumor necrosis factorɑ(TNF-ɑ)]and immunoglobulin E(IgE)before and after treatment and adverse reactions were compared between the two groups.Results:After treatment,the total effective rate in Chinese-western medicine group was 95.35%,which was higher than 81.40%in simple western medicine group(P<0.05).After treatment,the scores of clinical symptoms(wheal number,size and duration,pruritus)in both groups were decreased(P<0.05),and the scores in Chinese-western medicine group were lower than the simple western medicine group(P<0.05).After treatment,the scores of UAS and DLQI were reduced in both groups(P<0.05),and the scores were lower in Chinese-western medicine group than those in simple western medicine group(P<0.05).The level of serum C3 in both groups after treatment was enhanced(P<0.05),and the C3 level in Chinese-western medicine group was higher than that in simple western medicine group(P<0.05).After treatment,the levels of IL-4,IgE and TNF-ɑin the two groups were declined(P<0.05),and the levels were lower in Chinese-western medicine group than those in simple western medicine group(P<0.05).There was no statistical significance in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion:Jinchan Zhiyang capsules combined with levocetirizine has an exact efficacy in the treatment of patients with CU,and it can effectively relieve the symptoms,enhance the levels of serum complements and reduce the levels of inflammatory factors.
