摘　　要：目的分析痛风急性发作的危险因素。方法4649例痛风患者根据是否处于急性发作期分为急性组(302例,痛风急性期)和慢性组(4347例,痛风慢性期),采用单因素分析法及多因素Logistic回归分析法分析痛风急性发作的相关因素和危险因素。结果两组血清γ-谷氨酰转移酶(GGT)、门冬氨酸氨基转移酶(AST)、总胆红素、直接胆红素、间接胆红素、血糖(GLU)、甘油三酯(TG)、血尿素氮(BUN)、肌酐(CREA)、尿酸、肾小球滤过率、合并痛风石比较,P均<0.05。多因素分析显示,高GGT、AST、GLU、TG、BUN、CREA及合并痛风石是痛风急性发作的独立危险因素(P均<0.05)。结论GGT、AST、GLU、TG、BUN、CREA及合并痛风石是痛风急性发作的危险因素。Objective To explore the risk factors of acute gout flares.Methods A total of 4649 patients with gout were divided into the observation group(302 cases,acute stage of gout)and control group(4347 cases,chronic stage of gout)according to whether they were in the acute stage of gout.Univariate analysis and multivariate Logistic regression analysis were used to explore the correlation factors and risk factors of acute gout flares.Results Significant differences were found in the serumγ-glutamyltransferase(GGT),aspartate aminotransferase(AST),total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IDBIL),blood glucose(GLU),triglyceride(TG),blood urea nitrogen(BUN),creatinine(CREA),uric acid(UA),glomerular filtration rate(eGFR)and combined tophi between these two groups(all P<0.05).Multivariate analysis showed that high GGT,AST,GLU,TG,BUN,CREA,and combined tophi were independent risk factors for acute gout attack(all P<0.05).Conclusion GGT,AST,GLU,TG,BUN,CREA and combined tophi are risk factors for acute gout flares.
