摘 要:目的为慢性肾小球肾炎等肾病变的早期定量诊断提供新的影像诊断依据。方法选取腹部弥散张量成像扫描的不同年龄阶段的成人男女性检查正常者60例,在图像后处理工作站划分肾皮质和肾髓质的感兴趣区,测量各项异性分数(FA)值和表观弥散系数(ADC)值,生成纤维示踪图像。比较不同侧别、性别、部位、年龄组之间的肾皮质和肾髓质的FA值、ADC值。结果左右侧、男女性和不同部位、不同年龄组的正常肾皮质和肾髓质的FA值、ADC值差异均无统计学意义(P>0.05),但是左肾的ADC值略高于右肾,肾上极的ADC值略高于中部、下极,男性的FA值略高于女性,女性的ADC值略高于男性,肾皮质和肾髓质的FA值、ADC值随着年龄增高而逐渐略有降低。正常肾皮质和肾髓质的FA值、ADC值差异均有统计学意义(P<0.05),肾髓质的FA值高于肾皮质,肾皮质的ADC值高于肾髓质。结论弥散张量成像可以反映正常肾结构的特点,为肾及其病变的微细结构变化提供了客观影像诊断依据。Objective To provide a new imaging diagnostic basis for the early quantitative diagnosis of chronic glomerulonephritis and other renal diseases.Methods Sixty adult male and female of check for normality at different age stages of the abdominal diffusion tensor imaging scanning were selected.The regions of interest of the re⁃nal cortex and renal medulla were divided at the image post⁃processing workstation,the fractional anisotropy(FA)value and apparent diffusion coefficient(ADC)value were measured and generated fiber tracking images.The FA value and ADC value of the renal cortex and renal medulla were compared between the different sides,gender,loca⁃tion,age groups.Results There was no significant difference in FA value and ADC value of the normal renal cor⁃tex and renal medulla between the left and right sides,men and women,different parts,different age groups(P>0.05).However,the ADC value of the left kidney was slightly higher than that of the right kidney,the ADC value of the upper pole of the kidney was slightly higher than that of the middle part and lower pole,the FA value of men was slightly higher than that of the women,and the ADC value of women was slightly higher than that of the men,the FA value and ADC value of the renal cortex and renal medulla gradually decrease slightly with increase in age.There was significant difference in FA value and ADC value between the normal renal cortex and renal medulla(P<0.05),the FA value of renal medulla was higher than that of the renal cortex,and the ADC value of the renal cortex was higher than that of the renal medulla.Conclusion Diffusion tensor imaging can reflect the characteristics of normal renal structure,and provide an objective imaging diagnostic basis for the changes in the microstructures of the kid⁃ney and its lesions.
