针刀镜治疗肩周炎30例临床观察

摘　　要：目的:观察针刀镜治疗原发性肩关节周围炎的临床疗效。方法:将符合纳入排除标准的60例肩周炎患者随机分为针刀组和针刀镜组,每组各30例,针刀组采用针刀治疗,针刀镜组采用针刀镜治疗。分别在治疗前及治疗后2周、4周采用视觉疼痛模拟量表(VAS)评价肩关节疼痛改变程度,肩关节功能(Constant-Murley)评分评价两组肩关节活动改善程度,BI评分评价生活自理能力程度。总结两种方法治疗4周后的总有效率,以观察针刀镜治疗肩周炎的临床疗效。结果:治疗前两组的VAS评分、Constant-Murley评分及BI评分差异无统计学意义(P>0.05);治疗2周、4周后,两组VAS评分低于治疗前,Constant-Murley、BI评分高于治疗前,针刀镜组VAS评分低于针刀组,但组间对比没有统计学差异;Constant-Murley、BI评分高于针刀组,差异有统计学意义(P<0.01);且针刀镜组总有效率优于针刀组。结论:与针刀相比,针刀镜能更安全、有效的降低肩周炎患者疼痛,改善患者肩关节活动功能,远期效果较好,利于患者的康复。Objective To observe the clinical efficacy of Acupotomy Arthroscope in the treatment of primary periarthritis of shoulder.Methods 60 patients with periarthritis of shoulder that met the inclusion and exclusion criteria were divided into two groups by random number table method,30 cases in the acupotomology group with the treatment of acupotomology,30 cases in the Acupotomy Arthroscope group with the treatment of Acupotomy Arthroscope.Visual analog scale(VAS)was used to evaluate the degree of shoulder pain,Constant Murley score was used to evaluate the improvement of shoulder joint activity,and BI score was used to evaluate the level of self-care ability before treatment and 2 and 4 weeks after treatment.The total effective rate of the two groups after 4 weeks of treatment was summarized,thus to observe the clinical efficacy of AcupotomyArthroscope in the treatment of periarthritis of shoulder.Results Before treatment,there were no significant differences in VAS score,constant-Murley score and BI score between two groups(P>0.05).After 2 and 4 weeks of treatment,VAS score of two groups was lower than before,constant-Murley and BI scores were higher than before,and VAS score of Acupotomy Arthroscope group was lower than acupotomy group,but there was no statistical difference between two groups.Constant-murley and BI scores were higher than those in acupotomy group,and the differences were statistically significant(P<0.01).And the total effective rate of Acupotomy Arthroscope group was better than that of acupotomy group.Conclusion Compared with acupotomy,Acupotomy Arthroscope can safely and effectively reduce the pain of patients with periarthritis of shoulder,improve the shoulder joint activity function,and the long-term effect is better,it is beneficial to the rehabilitation of patients.
