深部浸润型子宫内膜异位症的阴道超声诊断价值探究

摘　　要：目的探讨经阴道超声诊断深部浸润型子宫内膜异位症的临床应用价值。方法回顾性分析2020年5月至2021年5月我院诊治的123例深部浸润型子宫内膜异位症患者的临床资料,将阴道超声检查结果与病理诊断结果进行对比,详细分析阴道超声诊断深部浸润型子宫内膜异位症的敏感度、特异度等。结果病理诊断为深部浸润型子宫内膜异位症123例,累及子宫直肠陷凹101例,累及阴道29例,累及直肠阴道隔24例,累及直肠87例。阴道超声诊断为深部浸润型子宫内膜异位症107例,检出率87.0%,累及子宫直肠陷凹73例,累及阴道23例,累及直肠阴道隔11例,累及直肠68例,超声诊断对于累及子宫直肠陷凹和累及直肠病变的检出效率最高(P<0.05)。阴道超声诊断和病理诊断的结果具有高度的一致性(P>0.05)。阴道超声对累及子宫直肠陷凹的敏感度为72.3%,累及阴道的敏感度为79.3%,累及直肠阴道隔的敏感度为45.8%,累及直肠的敏感度为78.2%;阴道超声对累及子宫直肠陷凹的特异度为81.8%,累及阴道的特异度为92.6%,累及直肠阴道隔的特异度为87.9%,累及直肠的特异度为91.7%。阴道超声对深部浸润型子宫内膜异位症不同受累部位的敏感度、特异度均较好。结论经阴道超声诊断深部浸润型子宫内膜异位症准确率高,可为临床诊疗提供可靠数据。Objective To investigate the application value of transvaginal ultrasonography in the diagnosis of deep infiltrating endometriosis.Methods From May 2020 to May 2021,123 patients with deep invasive endometriosis admitted were collected.Results of transvaginal ultrasonography were compared with the pathological diagnosis,sensitivity and specificity of transvaginal ultrasonography were analyzed to diagnosis of deep invasive endometriosis.Results Pathological diagnosis of deep infiltrating endometriosis was in this 123 cases,involving the hysterorectal lacunae 101 cases,the vagina 29 cases,the rectovaginal septum 24 cases,the rectum 87 cases.107 cases of deep infiltrating endometriosis were diagnosed by transvaginal ultrasonography,with a detection rate of 87.0%,involving the hysterorectal lacunae 73 cases,the vagina 23 cases,the rectovaginal septum 11 cases,and the rectum 68 cases.The detection rate of involving hysterorectal lacunae and rectal lesions by ultrasonic diagnosis was the highest(P<0.05).The results of transvaginal ultrasound diagnosis and pathological diagnosis were highly consistent(P>0.05).The sensitivity of transvaginal ultrasound to involvement of the hysterorectal lacunae was 72.3%,to involvement of the vagina 79.3%,to involvement of the rectovaginal septum 45.8%,and to involvement of the rectum 78.2%.The specificity of transvaginal ultrasound was 81.8%for involvement of the hysterorectal lacuna,92.6%for involvement of the vagina,87.9%for involvement of the rectovaginal septum,and 91.7%for involvement of the rectum.The sensitivity and specificity of transvaginal ultrasound to different affected sites of deep infiltrating endometriosis were preferably.Conclusion Transvaginal ultrasonography would have high accuracy in diagnosing deep infiltrating endometriosis and provide reliable data for diagnosis and treatment.
