摘 要:目的:探究肺表面活性物质(PS)与盐酸氨溴索雾化给药对早产儿肺透明膜病(HMD)的预防作用,比较PS与盐酸氨溴索两种药物的预防效用。方法:选取2018年1月—2020年9月湖北航天医院符合纳入标准的104例早产儿作为研究对象。依据随机数表法分为观察组和对照组,每组各52例。其中观察组使用PS为患儿治疗,对照组应用盐酸氨溴索雾化给药为患儿治疗。治疗期结束后观察两组早产儿的血气分析指标pH值、动脉氧分压(PaO_(2))、动脉二氧化碳分压(PaCO_(2))、氧合指数(FiO_(2))、发病情况、发病率及并发症发病率变化情况。结果:治疗前,各项指标比较,差异无统计学意义(P>0.05);治疗后,观察组的HMD发病例数和发病率较对照组显著降低,差异有统计学意义(χ^(2)=5.216,P<0.05);观察组pH值较对照组显著更高,差异有统计学意义(t=3.452,P<0.05);观察组的PaO_(2)值较对照组显著更高,但PaCO_(2)、FiO_(2)值均低于对照组,差异有统计学意义(t=10.820、4.897、8.025,P<0.05)。结论:应用PS和盐酸氨溴索雾化给药对于早产儿HMD具备很强的预防作用,可显著降低HMD发病率及并发症发病可能。Objective:To investigate the analysis of the preventive effect of pulmonary surface active substance(PS)and amiloride hydrochloride nebulized administration on pulmonary hyaline membrane disease(HMD)in preterm infants,and to compare the preventive utility of both PS and amiloride hydrochloride.Methods:104 preterm infants who met the inclusion and exclusion crite⁃ria in the hospital from January 2018 to September 2020 were selected,and the observation and control groups were selected based on the random number table method,with 52 cases in each group.The observation group was treated with PS and the control group was treated with nebulized administration of amiloride hydrochloride.After the end of the treatment period,the changes in the blood gas analysis indexes pH,arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),oxygenation index(FiO_(2)),morbidity,morbidity rate and complication rate were observed in the two groups of preterm infants.Results:Before treat⁃ment,the differences in the indicators were not statistically significant(P>0.05).After treatment,the number of HMD cases and morbidity rate were significantly lower in the observation group compared with the control group,and the difference was statistical⁃ly significant(χ^(2)=5.216,P<0.05).The pH values were significantly higher in the observation group compared to the control group,with a statistically significant difference(t=3.452,P<0.05).PaO_(2)values were significantly higher in the observation group com⁃pared to the control group,and PaCO_(2),FiO_(2)values were lower than the control group,with statistically significant differences(t=10.820,4.897,8.025,P<0.05).Conclusion:Nebulized administration of PS and amiloride hydrochloride has a strong preventive effect on HMD in preterm infants,significantly reducing the incidence of HMD and the possibility of complications.
