摘  要:目的:探讨回顾性心电门控技术联合大螺距扫描在主动脉扫描中提高主动脉根窦部(冠状动脉开口及瓣膜部)显示及图像质量的临床应用价值。方法:回顾性分析2019年6月—2020年5月宁夏回族自治区人民医院46例行主动脉CT血管造影(CTA)检查患者的影像资料,所有患者随机分为A组、B组,每组各23例。A组正常进行胸部主动脉增强扫描,B组在常规扫描的基础上加用回顾性心电门控技术行增强扫描。两组患者扫描管电压均为120 kV,采用自动毫安秒技术。观察两组患者主动脉根窦部(冠状动脉开口及瓣膜部)的清晰显示率并由2名高年资放射科医生主观评价其图像质量。测量主动脉根部,左、右冠状动脉开口,瓣膜处三处图像信噪比(SNR),对比度噪声比(CNR),比较两组患者图像中主动脉根窦部(冠状动脉开口及瓣膜部)显示情况。结果:观察组与对照组主动脉根部,左、右冠状动脉开口,瓣膜、斜方肌CT值比较,差异无统计学意义(P>0.05)。观察组与对照组主动脉根部,左、右冠状动脉开口,瓣膜SNR值及CNR值比较,差异无统计学意义(P>0.05)。结论:加用回顾性心电门控序列增强扫描获得的主动脉根窦部(冠状动脉开口及瓣膜部)图像质量明显优于普通增强扫描组,同时对图像SNR值及CNR值无明显影响。因此,针对主动脉根窦部(冠状动脉开口及瓣膜部)的扫描,可以选用回顾性心电门控序列扫描,以获得更加优质的图像。Objective:To investigate the clinical application value of retrospective cardiac gating technique combined with large pitch scanning in aortic scanning to improve the display and image quality of the aortic root sinus section(coronary artery opening and valve section).Methods:The imaging data of 46 patients who underwent CT angiography(CTA)of the aorta in the sample hospitals from June 2019 to May 2020 were retrospectively analyzed,and all patients were randomly divided into group A and group B,with 23 patients in each group.Group A underwent normal thoracic aortic enhancement scans,while group B under⁃went enhancement scans with retrospective cardiac gating technique in addition to the conventional scans.The scanning tube voltage was 120 kV in both groups of patients,and the automatic milliamperesecond technique was used.The image quality of the aortic root sinus(coronary artery opening and valve)was observed and subjectively evaluated by two senior radiologists in both groups.The signal-to-noise ratio(SNR)and the contrast-to-noise ratio(CNR)were measured at the aortic root,the left and right coronary openings,and the valve.The images of the aortic root sinus(coronary artery opening and valve)were compared between the two groups of patients.Results:There was no statistically significant difference between the CT values of the aortic root,left and right coronary artery openings,valves,and oblique muscles in the observation group and the control group(P>0.05).There was no statistically significant difference between the aortic root,left and right coronary artery openings,valve SNR values and CNR values in the observation group and the control group(P>0.05).Conclusion:The image quality of the aortic root sinus section(coronary artery opening and valve section)obtained by adding retrospective ECG gated sequence enhancement scan was significantly better than that of the plain enhancement scan group,while there was no significant effect on the image SNR and CNR values.Therefore,for scanning of the aortic root sinus(
