软通道钻孔引流术联合腰大池置管对非脑疝高血压性脑出血合并蛛网膜下腔出血的疗效

摘　　要：目的:探究软通道钻孔引流术联合腰大池置管对非脑疝高血压性脑出血(HICH)合并蛛网膜下腔出血的疗效。方法:选取2020年1月—2022年8月菏泽医学专科学校附属医院收治的非脑疝HICH合并蛛网膜下腔出血患者62例作为研究对象,随机分为观察组与对照组,各31例。对照组接受单纯软通道钻孔引流术治疗,观察组在对照组基础上联合腰大池置管治疗。比较两组患者临床疗效、不良反应及格拉斯哥预后评分(GOS)。结果:观察组总有效率高于对照组,差异有统计学意义(P=0.042);观察组不良反应发生率低于对照组,差异有统计学意义(P=0.001);观察组术后1、2、3个月GOS评分高于对照组,差异有统计学意义(P<0.05)。结论:软通道钻孔引流术联合腰大池置管对非脑疝HICH合并蛛网膜下腔出血的疗效显著,能够有效提高患者神经功能,改善GOS评分,降低不良反应发生率,有利于治疗及预后。Objective:To explore the effect of soft-channel-drilling-drainage therapy combined with continuous lumbar cerebro⁃spinal fluid drainage(CLCFD)in patients with non-cerebral hernation hypertensive intracerebral hemorrhage(HICH)complicated with subarachnoid hemorrhage.Methods:62 patients with above mentioned diseases in Affiliated Hospital of Heze Medical Col⁃lege from January 2020 to August 2022 were divided into the observation group and the control group with 31 cases in each group.The control group received only soft channel drilling-drainage therapy,while the observation group received soft channel drilling and drainage combined with CLCFD.Clinical efficacy,side effects,and GOS score of both groups were recorded and compared af⁃ter the surgery.Results:The recovery rate and effective rate of observation group were significantly higher than control group(P=0.042).The incidence of adverse reactions in observation group was significantly lower than that in control group(P=0.001).The GOS score of observation group was significantly higher than that of control group at 1 month,2 month and 3 months after opera⁃tion(P<0.05).Conclusion:Soft channel drilling-drainage combined with CLCFD in the treatment of non-cerebral hernia HICH complicated with subarachnoid hemorrhage is effective,effectively reduce complications,improve the neurological function of pa⁃tients,improve GOS score,is beneficial to treatment and prognosis,worthy of clinical promotion.
