摘 要:目的探究促红细胞生成素(EPO)对早产儿血清S-100β水平及振幅整合脑电图(aEEG)参数、颅脑磁共振成像(MRI)影像指标的影响。方法前瞻性选取2021年1月至12月海南医学院第一附属医院收治的早产儿95例,按照随机数字表法将患儿分为对照组(n=48)和观察组(n=47)。对照组患儿采用胞二磷胆碱治疗,0.125 g/d,观察组采用EPO治疗,250 IU·kg^(-1)·d^(-1),治疗周期2周。比较两组患儿治疗2周后的血清S-100β水平、神经行为测定(NBNA)评分、各aEEG参数。胎龄纠正40周时,两组患儿行颅脑MRI影像检查,计算T_(1)WI信号强度比值(G/P)。随访1年,比较两组患儿的丹佛发育筛查表(DDST)结果。结果治疗2周后,观察组患儿血清S-100β水平为(2.51±1.05)ng/L,低于对照组[(4.56±1.23)ng/L],差异有统计学意义(P<0.05)。治疗2周后,观察组患儿的NBNA评分(37.51±1.23)分,高于对照组[(35.56±1.03)分],校正后aEEG总分为(2.31±0.13)分,低于对照组[(2.45±0.23)分],差异均有统计学意义(P<0.05)。胎龄纠正40周后,观察组G/P为1.17±0.04,低于对照组(1.27±0.07),差异有统计学意义(P<0.05)。随访1年后,观察组患儿的DDST评分为(89.51±7.20)分,高于对照组[(82.56±8.46)分],差异有统计学意义(P<0.05)。结论EPO治疗可降低早产儿的血清S-100β水平、aEEG参数及G/P,促进早产儿的神经和智力发育,对于早产儿脑损伤具有一定的保护作用,值得在临床推广应用。Objective To explore the effects of erythropoietin(EPO)on serum S-100β level,amplitude integrated electro encephalogram(aEEG)parameters and brain magnetic resonance imaging(MRI)image indexes of preterm infants.Methods A total of 95 premature infants admitted to the First Affiliated Hospital of Hannan Medical University from January to December 2021 were selected and divided into the control group(n=48)and the observation group(n=47)according to the random number table method.The control group was treated with citicoline at 0.125 g/d,while the observation group was treated with EPO at 250 IU·kg^(-1)·d^(-1) and the treatment cycle lasted for 2 weeks.The serum S-100β levels,neurobehavioral assessment(NBNA)scores,and various aEEG parameters of the two groups of children after 2 weeks of treatment were compared.At 40 weeks of corrected gestational age,both groups of children underwent cranial MRI imaging,and the T_(1)WI signal intensity ratio(G/P)was calculated.After a one-year follow-up,the results of the Denver Developmental Screening Table(DDST)were compared between the two groups of children.Results After 2 weeks of treatment,the serum S-100βlevel in the observation group was(2.51±1.05)ng/L,which was significantly lower than that in the control group[(4.56±1.23)ng/L],the difference was statistically significant(P<0.05).After 2 weeks of treatment,the NBNA score of the observation group was(37.51±1.23)points,which was higher than that of the control group(35.56±1.03)points,the total aEEG score after correction was(2.31±0.13)points,which was lower than that of the control group(2.45±0.23)points,the differences were statistically significant(P<0.05).After correcting the gestational age for 40 weeks,the G/P in the observation group was 1.17±0.04,which was lower than that in the control group(1.27±0.07),and the difference was statistically significant(P<0.05).After one year of follow-up,the DDST score of the observation group was(89.51±7.20)points,which was higher than the control group(82.56±8.46)points,
