摘　　要：目的探究通络止痛散外用联合黄藤浸膏片、驱寒通痹片治疗类风湿关节炎(RA)患者的疗效及对其相关实验室指标的影响。方法将2020年1月至2023年1月洪湖市中医医院收治的100例RA患者纳入本次前瞻性研究,按照随机数字表法将其分为研究组(n=50)和对照组(n=50)。研究组患者采用通络止痛散外用+黄藤浸膏片+驱寒通痹片治疗,对照组患者采用黄藤浸膏片+驱寒通痹片治疗,疗程6周。比较2组患者的临床疗效、治疗6周后相关实验室指标[丙氨酸氨基转移酶(ALT)、天门冬酸氨基转移酶(AST)、尿素氮、肌酐和类风湿因子(RF)、抗CCP抗体及C反应蛋白(CRP)、红细胞沉降率(ESR)]、视觉模拟评分法(VAS)评分和28个关节疾病活动性得分(DAS28)评分及不良反应发生情况。结果研究组的总有效率为96.00%,显著高于对照组(80.00%),差异有统计学意义(P<0.05)。治疗6周后,研究组与对照组的肌酐、尿素氮、ALT、AST水平比较,差异均无统计学意义(P>0.05)。治疗6周后,研究组的RF、抗CCP抗体、ESR与对照组比较,差异均无统计学意义(P>0.05),但研究组的血清CRP为(11.63±5.26)mg/L,显著低于对照组[(14.52±5.71)mg/L],差异有统计学意义(P<0.05)。治疗6周后,研究组的VAS评分、DAS28评分分别为(2.03±0.68)、(2.61±0.54)分,均显著低于对照组[(2.95±0.81)、(3.05±0.61)分],差异均有统计学意义(P<0.05)。研究组不良反应发生率为18.00%,与对照组的14.00%相比,差异无统计学意义(P>0.05)。结论通络止痛散外用联合黄藤浸膏片、驱寒通痹片治疗RA的疗效良好,能有效缓解病情和减轻疼痛,改善生化指标,安全性高。Objective To explore the efficacy of Tongluo Zhitong Powder for external use combined with Huangteng extract tablets and Quhan Tongbi tablets in the treatment of rheumatoid arthritis(RA)patients and its impact on relevant laboratory indicators.Methods A total of 100 patients with RA treated in Honghu Traditional Chinese Medicine Hospital from January 2020 to January 2023 were included in this prospective study,and were divided into the study group(n=50)and control group(n=50)according to the random number table.The study group patients were treated with Tongluo Zhitong powder for external use,Huangteng Extract Tablets,and Quhan Tongbi tablets for 6 weeks,the control group patients were given Huangteng Extract tablets and Quhan Tongbi tablets for 6 weeks.The clinical efficacy,related laboratory indicators[including alanine aminotransferase(ALT),aspartate aminotransferase(AST),urea nitrogen,creatinine,rheumatoid factor(RF),anti CCP antibody(CCP-AB)and C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)],visual analog scale(VAS)score,disease activity score 28(DAS28)score after 6 weeks of treatment and adverse reactions were compared.Results The total effective rate of the study group was 96.00%,which was significantly better than that of the control group(80.00%),the difference was statistically significant(P<0.05).After 6 weeks of treatment,there were no statistically significant differences in the levels of creatinine,urea nitrogen,ALT,and AST between the study group and the control group(P>0.05).After 6 weeks of treatment,there was no statistically significant difference in RF,anti CCP antibodies,and ESR between the study group and the control group(P>0.05),but the serum CRP of the study group was(11.63±5.26)mg/L,which was significantly lower than the control group[(14.52±5.71)mg/L],the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was 18.00%,and there was no statistically significant difference compared to the 14.00%in the control group(P>0.05).C
