疏密波电针针刺下肢关键肌及神经干穴位治疗不完全脊髓损伤的效果  

Effect of dense-sparse waves eletroacupuncture on key muscle of lower limbs and nerve trunk acupoints in the treatment of incomplete spinal cord injury

摘　　要：目的分析疏密波电针针刺下肢关键肌及神经干穴位对不完全脊髓损伤患者下肢运动功能及血清学指标的影响。方法选择不完全脊髓损伤患者56例,采用随机数表法分为对照组与观察组,各28例。对照组应用连续波电针,采用传统普通针灸取穴,观察组应用疏密波电针针刺下肢关键肌及神经干穴位治疗,2组均治疗4周。比较2组治疗4周后疗效,治疗前、治疗4周后下肢运动功能、生活质量、心理应激、血清学指标、肌电图指标,治疗期间不良反应发生情况。结果治疗4周后,观察组总有效率(89.29%,25/38)高于对照组(64.29%,18/38)(P<0.05)。与治疗前比较,治疗4周后,2组下肢美国脊髓损伤协会(ASIA)功能、Berg平衡量表、改良Barthel指数(MBI)评分,Cortical波、Lsp波波幅,均方根值(RMS)升高,且观察组高于对照组(P<0.05)。与治疗前比较,治疗4周后,2组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,血清C反应蛋白(CRP)、降钙素原(PCT)水平,Cortical波、Lsp波的潜伏期均降低,且观察组低于对照组(P<0.05)。与治疗前比较,治疗4周后,2组改良Ashworth量表评分均升高,观察组升高幅度低(P<0.05)。治疗期间,2组不良反应发生率差异无统计学意义(P>0.05)。结论疏密波电针针刺下肢关键肌及神经干穴位,可改善不完全脊髓损伤患者下肢运动功能,提高生活质量,减轻心理应激,疗效确切,安全性良好,可能与调节血清学及肌电图指标有关。Objective To analyze the effects of dense-sparse waves electroacupuncture at key muscle of lower limbs and nerve trunk acupoints on motor function of lower limbs and serological indexes in patients with incomplete spinal cord injury.Methods A total of 56 patients with incomplete spinal cord injury were selected and divided into the control group and the observation group according to random number table method,with 28 cases in each group.The control group was treated with continuous wave electroacupuncture and traditional ordinary acupuncture point selection,while the observation group was treated with dense-sparse waves electroacupuncture at key muscle of lower limbs and nerve trunk acupoints.Both groups were treated for 4 weeks.The therapeutic efficacy after 4 weeks of treatment,motor function of lower limbs,quality of life,psychological stress,serological indexes and electromyography indexes before treatment and after 4 weeks of treatment,and the occurrence of adverse reactions during treatment were compared between the two groups.Results After 4 weeks of treatment,the total effective rate of the observation group(89.29%,25/38)was higher than that of the control group(64.29%,18/38)(P<0.05).Compared with before treatment,after 4 weeks of treatment,the scores of lower limb function of American Spinal Injury Association(ASIA)and Berg Balance Scale,and improved Barthel Index(MBI),and Cortical waves and Lsp waves amplitude,and root mean square(RMS)value in the two groups increased,and the observation group was higher than the control group(P<0.05).Compared with before treatment,after 4 weeks of treatment,the scores of Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS),and serum levels of C-reactive protein(CRP)and procalcitonin(PCT)in the two groups decreased,and latency of Cortical waves and Lsp waves in the two groups was shortened,and the observation group was lower than the control group(P<0.05).Compared with before treatment,after 4 weeks of treatment,the scores of modified Ashworth scale in b
