归脾汤加减治疗特发性瘦削临证经验

摘　　要：特发性瘦削是指不明原因导致的肌肉消瘦,多因后天脾胃失其健运,气血亏虚不能充养机体所致。临床表现除机体消瘦、体质量指数相对较低外,其他症状表现较为隐蔽,如易受惊吓、易失眠、易怒、不耐寒冷、易外感、易乏力、记忆力较差、多食或少食,此类症状较轻,易忽视而延误治疗。特发性瘦削日久易发展为正气不足而体弱多病,对环境适应能力降低,且易引发它病,尤其外感疾病。临床采用归脾汤加减,健运后天脾胃以生气血、充养周身、厚肌肉,改善瘦削体质,增强正气,预防恶病候产生。临床研究亦发现,部分患儿有先天不足之因素,可配合六味地黄丸服用,以先后天互资,滋养筋骨肌肉。从脾论治特发性瘦削,有助于改善人体症状和体质,以及预防肿瘤等恶病候的发生。Idiopathic emaciation refers to unexplained muscle wasting,mostly caused by acquired spleen and stomach losing its health,blood deficiency failing to fill the body.In addition to body weight loss and relatively low body mass index,other clinical manifestations are more subtle,such as susceptibility to shock,insomnia,irritability,intolerance to cold,susceptibility to external sensations,weakness,poor memory,overeating or undereating.This type of symptoms is relatively mild and can be easily ignored and delayed treatment.Long time idiopathic emaciation is prone to develop into deficiency of vital energy and physical weakness,reduced adaptability to the environment,and easy to cause other diseases,especially exogenous diseases.Clinical use of modified Guipi Decoction can invigorate the acquired spleen and stomach to generate qi and blood,replenish and nourish the whole body,thicken muscles,improve lean physique,strengthen healthy qi,and prevent the occurrence of cachexia.Clinical studies have also found that some children have congenitally deficient factors,which can be combined with Liuwei Dihuang Pills to nourish muscles.The treatment of idiopathic emaciation from the spleen helps to improve human symptoms and physique,and prevent the occurrence of malignant diseases such as tumors.
