摘  要:目的探究马拉色菌感染与寻常型银屑病进展的关系及其可能的潜在机制,为临床治疗提供依据.方法选取2018年8月-2021年12月于杭州市第三人民医院就诊的马拉色菌感染寻常型银屑病患者40例为感染组,同期40例未感染马拉色菌寻常型银屑病患者为未感染组.比较两组患者一般资料、病情程度、视觉模拟量表(VAS)评分、NLR家族含pyrin结构域蛋白3(NLRP3)炎症小体水平、炎症因子及氧化应激指标水平.结果感染组重度患者占比、VAS评分均高于未感染组(P<0.05);感染组皮损处的NLRP3水平及血清白细胞介素(IL)-1β及IL-18水平均高于未感染组(P<0.05);感染组皮损处氧化应激指标丙二醛(MDA)水平高于未感染组,过氧化物歧化酶(SOD)及谷胱甘肽过氧化物酶(GSH-Px)水平低于未感染组(P<0.05).结论寻常型银屑病患者合并马拉色菌感染与重度病情风险相关,其机制可能与NLRP3炎症小体活化引起的炎症因子释放及氧化应激两方面有关.OBJECTIVE To observe the association between Malassezia infection and progression of psoriasis vulgaris and analyze its possible mechanisms so as to provide guidance for clinical treatment.METHODS:A total of 40 psoriasis vulgaris patients with Malassezia infection who were treated in the Third People's Hospital of Hangzhou from Aug 2018 to Dec 2021 were assigned as the infection group,meanwhile,40 psoriasis vulgaris patients without Malassezia infection were chosen as the no infection group.The baseline data,illness condition,visual analogue score(VAS)score,NLR family containing pyrin structural domain protein(NLRP3)inflammasome,inflammatory factors and oxidative stress indexes were observed and compared between the two groups of patients.RESULTS The proportion of severe patients and VAS score of the infection group were higher than those of the no infection group(P<0.05).The NLRP3 level in skin damage site,serum interleukin(IL)-1βand IL-18 levels of the infection group were higher than those of the no infection group(P<0.05);the oxidative stress index malonaldehyde level in skin damage site of the infection group was higher than that of the no infection group,while the levels of superoxide dismutase and glutathione peroxidase(GSH-Px)of the infection group were lower than those of the no infection group(P<O.05).CONCLUSION:The complication with Malassezia infection in the psoriasis vulgaris patients is associated with the risk of severe illness condition,and the mechanisms may be associated with the release of inflammatory factors induced by activation of NLRP3 inflammasome and oxidative stress.
