摘 要:目的探讨曲霉菌感染对类风湿关节炎患者巨噬细胞功能的影响及其调控机制。方法选取2020年9月-2022年8月在天津市第五中心医院就诊的类风湿关节炎患者81例为研究对象,根据患者肺部是否感染曲霉菌分为感染组33例与未感染组48例,另选同期健康体检者50名为健康组,检测入组研究对象痰液或呼吸道分泌物巨噬细胞占比、吞噬功能及相关受体[低氧诱导因子-1α(HIF-1α)、Bcl-2同源结构域蛋白抗体(Beclin)-1、微管相关蛋白轻链3(LC3)-I、LC3-II]mRNA与蛋白活化水平。结果感染组细胞总数高于未感染组和健康组,巨噬细胞占比低于未感染组和健康组(P<0.05);吞噬功能从高到低依次为健康组、感染组、未感染组(P<0.05);感染组HIF-1αmRNA及蛋白相对表达水平高于未感染组和健康组,Beclin-1、LC3-I、LC3-II mRNA及蛋白相对表达水平低于未感染组(P<0.05)。结论曲霉菌干预类风湿关节炎患者巨噬细胞的吞噬功能,通过激活HIF-1α达到促进巨噬细胞吞噬功能的目的,推测其可能为机体对抗病原体入侵的一种自身防御机制。OBJECTIVE To explore the influence of Aspergillus infection on macrophage function of the patients with rheumatoid arthritis and analyze the regulatory mechanisms.METHODS A total of 81 patients with rheumatoid arthritis who were treated in Tianjin Fifth Central Hospital from Sep 2020 to Aug 2022 were recruited as the research subjects and were divided into the infection group with 33 cases and the no infection group with 48 cases according to the status of pulmonary Aspergillus infection,meanwhile,5o healthy people who received physical examination were chosen as the healthy group.The percentage of macrophages in sputum specimens or respiratory tract secretions,phagocytic function and levels of related receptors[hypoxia-inducible factor-lα(HIF-1α),Bcl-2 homeodomain protein antibody(Beclin-1),microtubule-associated protein light chain 3Ⅱ(LC3-Ⅱ),microtubule-associated protein light chain 3Ⅱ(LC3-Ⅱ)JmRNA and protein activation were detected for the research subjects.RESULTS The total counts of cells of the infection group were higher than those of the no infection group and the healthy group(P<0.05).The percentage of macrophage of the infection group was lower than that of the no infection group and the healthy group.The phagocytic function,from high to low,were in order as follows:the healthy group,the infection group,the no infection group(P<0.05).The relative expression levels of HIFlamRNA and protein of the infection group were higher than those of the no infection group and the healthy group,while the relative expression levels of Beclin-1,LC3-Ⅱ,LC3-Ⅱ mRNA and proteins of the infection group were lower than those of the no infection group(P<O.05).CONCLUSION Aspergillus intervenes in the phagocytic function of macrophage of the rheumatoid arthritis patients and promotes the phagocytic function of macrophage by activating hypoxia-inducible factor la,indicating that it may be a self-defense mechanism against the invasion of pathogens.
