摘　　要：目的观察单侧外固定架和环状外固定架治疗股骨慢性骨髓炎骨缺损的疗效.方法选择2020年1月-2022年7月首都医科大学附属北京康复医院骨科二病区收治的股骨慢性骨髓炎骨缺损患者120例为研究对象,根据使用的固定架分为单边祖与环形组,每组均60例.单边组采用单边外固定架,环形组采用Ilizamv环形外固定架.比较两组术后治疗效果.结果单边组与环形组术后并发症发生率(钉道感染、牵拉区域软组织疼痛、一期骨性愈合)比较无统计学差异;愈合时间、骨愈合指数、骨延长长度、固定架固定时间及固定指数比较差异均无统计学意义;两组患者固定期间膝关节活动受限程度比较差异有统计学意义,其中环形组膝关节活动受限程度在16°~30°的占比低于单边组,0°~15°的占比高于单边组(Z=3.214,P=0.012);单边组治疗优良率为96.7%,环形组治疗优良率为100.0%,两组比较差异无统计学意义(χ^(2)=2.034,P=0.154).结论单侧外固定架和环状外固定架治疗股骨慢性骨髓炎骨缺损具有较好的效果,且可有效地抑制炎症,促进骨和软组织的愈合.OBJECTIVE To observe the effects of unilateral external fixation and circular external fixation on treatment of the patients with chronic osteomyelitis of femur and bone defects.METHODS A total of 120 patients with chronic osteomyelitis of femur and bone defects who were treated in second ward of orthopedics department of Beijing Rehabilitation Hospital Affiliated to Capital Medical University from Jan 2020 to Jul 2022 were recruited as the research subjects and were divided into the unilateral group and the circular group according to the type of fixation frame with 60 cases in each group.The unilateral group was treated with unilateral external fixation frame,and the circular group was treated with Ilizamv circular external fixation frame.The postoperative therapeutic effects were observed and compared between the two groups.RESULTS There was no significant difference in the incidence of postoperative complications(nail infection,soft tissue pain in traction area,phase I bone healing)between the unilateral group and the circular group.There were no significant differences in the healing time,bone healing index,bone elongation length,fixation time of fixation frame and fixation indexes between the two groups.There was significant difference in the limitation of motion(LOM)of knee joint between the two groups of patients during the fixation,the proportion of the patients with LOM of knee joint ranging between 16°and 30°was higher in the circular group than in the unilateral group,and the proportion of the patients with LOM of knee joint ranging between 0°and 15°was higher in the circular group than in the unilateral group(Z=3.214,P=0.012).The rate of excellent and good treatment of the unilateral group was 96.7%,the circular group 100.0%,and there was no significant difference between the two groups(χ^(2)=2.034,P=0.154).CONCLUSION The unilateral external fixation and circular external fixation can achieve remarkable effects on treatment of the patients with chronic osteomyelitis of femur and bone defects,eff
