摘 要:目的分析肺炎支原体肺炎(MPP)患儿血小板反应蛋白1(TSP-1)、高迁移率族蛋白1(HMGB1)、正五聚蛋白3(PTX3)水平及其与肺炎支原体DNA水平的相关性.方法选取2020年1月-2022年7月许昌市妇幼保健院收治的MPP患儿135例作为研究组(轻症77例、重症58例),另选取同期于本院健康体检的儿童147名作为对照组,检测血清TSP-1、HMGB1、PTX3水平,并分析不同病情、不同病程MPP患儿血清TSP-1、HMGB1、PTX3水平、肺泡灌洗液肺炎支原体DNA水平.结果研究组血清TSP-1、HMGB1、PTX3水平高于对照组(P<0.05);重症患儿急性期和恢复期血清TSP-1、HMGB1、PTX3水平,肺泡灌洗液肺炎支原体DNA水平均高于轻症患儿(P<0.05);轻症、重症患儿急性期血清TSP-1、HMGB1、PTX3水平,肺泡灌洗液肺炎支原体DNA水平均高于恢复期(P<0.05);Pearson相关性分析结果显示,血清TSP-1、HMGB1、PTX3水平与肺炎支原体DNA水平呈正相关;Spearman相关性分析结果显示,血清TSP-1、HMGB1、PTX3水平与病情严重程度呈正相关.结论MPP患儿血清TSP-1、HMGB1、PTX3水平升高,且水平与病情严重程度、肺炎支原体DNA水平呈正相关,可作为儿童MPP临床诊治的新靶点.OBJECTIVE To analyze the levels of thrombospondin-1(TSP-1),high mobility group box 1(HMGB1)and pentraxin 3(PTX3)of the children with Mycoplasma pneumoniae pneumonia(MPP)and observe their association with M.pneumoniae deoxyribonucleic acid(DNA).METHODS A total of 135 children with MPP who were treated in Xuchang Maternity and Child Health Hospital from Jan 2020 to Jul 2022 were assigned as the study group(77 mild cases,58 severe cases).Meanwhile,147 children who received physical examination in the hospital were chosen as the control group.The levels of serum TSP-1,HMGB1 and PTX3 were detected.The levels of serum TSP-1,HMGBl and PTX3 as well as the M.pneumoniae DNA level in bronchoalveolar lavage fluid(BALF)were observed and compared among the children with different illness condition and courses.RESULTS The levels of serum TSP-1,HMGB-1 and PTX-3 of the study group were higher than those of the control group(P<0.05).The levels of serum TSP-1,HMGB1 and PTX3 as well as the level of bronchoalveolar lavage M.pneumoniae DNA of the severe children were higher than those of the mild children during the acute stage and recovery stage(P<0.05).The levels of serum TSP-1,HMGB1 and PTX3 as well as the level of bronchoalveolar lavage M.pneumoniae DNA of the mild,severe children were higher during the acute stage than during the recovery stage(P<0.05).Pearson correlation analysis showed that the serum TSP-1,HMGB1 and PTX3 levels were positively correlated with the M.pneumoniae DNA level.Spearman correlation analysis indicated that the serum TSP-1,HMGB1 and PTX3 were positively correlated with severity of disease.CONCLUSION The levels of serum TSP-1,HMGB1 and PTX3 of the children with MPP are elevated and are positively correlated with the severity of disease and the M.pneumoniae DNA level,which can be used as new targets for clinical diagnosis and treatment of the children with MPP.
