当归散联合补肾祛瘀方治疗肾虚血瘀型不明原因复发性自然流产临床观察

摘　　要：目的观察当归散联合补肾祛瘀方治疗肾虚血瘀型不明原因复发性自然流产(URSA)的临床疗效。方法收集2020年2月至2022年2月就诊的150例肾虚血瘀型URSA患者作为研究对象,按照随机数字表法分为2组,对照组75例予补肾祛瘀方治疗,治疗组75例在对照组的基础上联合当归散治疗。2组均治疗4周为1个疗程,治疗2个疗程后统计疗效,比较2组治疗前后血清白细胞介素17A(IL-17A)、IL-1β、IL-35、CC趋化因子受体4(CCR4)、CC趋化因子17(CCL17)及叉头框蛋白P3(FOXP3)水平变化情况,比较2组治疗前后性激素雌二醇(E_(2))、孕酮(P)及人绒毛膜促性腺激素(β-HCG)水平变化情况,比较2组治疗前后中医症状积分变化情况。结果治疗组总有效率94.67%(71/75),对照组总有效率81.33%(61/75),治疗组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清IL-17A及IL-1β水平均降低(P<0.05),IL-35、CCR4、CCL17及FOXP3水平均升高(P<0.05),且治疗组治疗后血清IL-17A及IL-1β水平均低于对照组(P<0.05),IL-35、CCR4、CCL17及FOXP3水平均高于对照组(P<0.05)。与本组治疗前比较,2组治疗后性激素E_(2)、P及β-HCG水平均升高(P<0.05),且治疗组治疗后性激素E_(2)、P及β-HCG水平均高于对照组(P<0.05)。与本组治疗前比较,2组治疗后中医症状总分均降低(P<0.05),且治疗组治疗后中医症状总分低于对照组(P<0.05)。结论当归散联合补肾祛瘀方治疗肾虚血瘀型URSA患者临床疗效确切,可有效改善患者中医症状,提高临床疗效,其作用机制可能与调节患者免疫功能、提高母体免疫耐受、提升性激素水平有关。Objective To analyze the effects of Danggui Powder combined with Bushen Quyu Formula on patients with unexplained recurrent spontaneous abortion(URSA)of kidney blood type.Methods A total of 150 URSA patients of kidney blood type admitted to our hospital from February 2020 to February 2022 were collected as the research objects.The patients were randomly grouped at 1:1 ratio.All patients was treated with Bushen Quyu Formula,the patients of the treatment group was additionally treated with Danggui Powder.Both groups were treated for 4 weeks(a course of treatment),and the curative effect was counted after 2 courses of treatment.The aim was to compared the levels of interleukin 17A(IL-17A),interleukin 1β(IL-1β),interleukin 35(IL-35),CC-chemokine receptor 4(CCR4),CC-chemokine ligand 17(CCL17),Forkhead box P3(FOXP3),estradiol(E_(2)),progesterone(P),β-chorionic gonadotropin(β-hCG).The scores of traditional Chinese medicine(TCM)syndromes were assessed.Results The total effective rate of the treatment group was significantly higher than that of the control group(94.67%[71/75]vs 81.33%[61/75],P<0.05).Significantly reduced IL-17A and IL-1β(P<0.05),and increased IL-35,CCR4,CCL17 and FOXP3(P<0.05)were detected in the both groups,and the treatment group was superior to the control group in the levels of IL-17A,IL-1β,IL-35,CCR4,CCL17 and FOXP3(all P<0.05).The levels of E_(2),P andβ-hCG in the both groups increased significantly after treatment(P<0.05),and which in the treatment group were significantly higher than those in the control group(P<0.05).The total scores of TCM symptoms in both groups reduced significantly after treatment(P<0.05),and which in the treatment group were more prevalent(P<0.05).Conclusion For URSA patients of kidney blood type,Danggui Powder combined with Bushen Quyu Formula has definite clinical efficacy.It can effectively improve patients'TCM symptoms and clinical efficacy.Its action mechanism may be related to regulating immune function,improving maternal immune tolerance,and increasing sex horm
