柴芩温胆汤加减联合注射用阿奇霉素治疗小儿肺炎支原体肺炎的临床观察及对患儿血清白细胞介素1β、白细胞介素8水平的影响

摘　　要：目的观察柴芩温胆汤加减联合注射用阿奇霉素治疗小儿肺炎支原体肺炎(MPP)的临床疗效,以及患儿血清白细胞介素1β(IL-1β)及IL-8水平的影响。方法选取2021年11月至2022年11月收治的小儿MPP患者60例为研究对象,按照随机数字表法分为2组,对照组30例予注射用阿奇霉素治疗,治疗组30例在对照组的基础上联合柴芩温胆汤加减治疗。2组均治疗5~7天后统计疗效,比较2组治疗前后炎症因子指标血清IL-1β及IL-8水平变化,比较2组治疗后发热、咳嗽及肺部啰音完全消失时间情况,比较2组治疗期间出现呕吐、头晕、腹痛等不良反应情况。结果治疗组总有效率96.67%(29/30),对照组总有效率76.67%(23/30),治疗组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后炎症因子指标血清IL-1β及IL-8水平均降低(P<0.05),且治疗组治疗后血清IL-1β及IL-8水平均低于对照组(P<0.05)。治疗组治疗后发热、咳嗽及肺部啰音完全消失时间均短于对照组,比较差异均有统计学意义(P<0.05)。治疗组治疗期间不良反应总发生率16.67%(5/30),对照组不良反应总发生率10.00%(3/30),2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论柴芩温胆汤加减联合注射用阿奇霉素治疗小儿MPP疗效确切,可有效改善患者临床症状,促进发热、咳嗽及肺部啰音消失,且安全可靠,无明显不良反应,其作用机制可能与降低血清IL-1β及IL-8水平,抑制炎性反应有关。Objective To explore the clinical efficacy of modified ChaiqinWendan Decoction combined with injection of azithromycinfor mycoplasma pneumoniae pneumonia(MPP)in children and its influence on serum interleukin 1β(IL-1β)and interleukin 8(IL-8).Methods From November 2021 to November 2022,a total of 60 MPP children were randomly assigned into the treatment group(n=30)and the control group(n=30).All patients were managed by injection of azithromycin,and those in the treatment group were additionally treated with modified ChaiqinWendan Decoction.The treatment was performed for 5~7 days,aiming to compare inflammatory factors(IL-1β,IL-8),disappearance time of fever,cough,pulmonary rale,adverse reactions(vomiting,dizziness,abdominal pain).The curative effect was assessed.Results The overall effective rate was significantly higher in the treatment groupthan that of the control group(96.67%[29/30]vs 76.67%[23/30],P<0.05).After treatment,IL-1β,IL-8 in groups were significantly decreased(P<0.05),the reduction in the treatment group was more prevalent compared with the control group(P<0.05).The disappearance time of fever,cough,pulmonary rale in the treatment group was significantly shorter than that in the control group(P<0.05).The difference was not statistically significant in the total incidence of adverse reactions between the treatment group and the control group(16.67%[5/30]vs 10.00%[3/30],[P>0.05],respectively).Conclusion For MPP children,modified ChaiqinWendan Decoction combined with azithromycin for injection has definite curative effect,which can effectively improve clinical symptoms,promote the disappearance of fever,cough,pulmonary rale by reducing serum IL-1βand IL-8 levels,inhibiting inflammatory reactions.It is safe and reliable,and with no obvious adverse reactions.
