自体表皮移植术联合308 nm准分子激光在稳定期白癜风治疗中的应用

摘　　要：目的 探讨自体表皮移植术联合308nm准分子激光在稳定期白癜风治疗中的治疗效果。方法 回顾性分析2019年1月~2022年10月在我院治疗的98例稳定期白癜风患者病例资料,根据治疗方法分为对照组(40例)和研究组(58例)。对照组采用自体表皮移植术,研究组采用自体表皮移植术联合308 nm准分子激光治疗。治疗3个月后比较两组临床效果、皮损区色素恢复程度、皮损区炎症因子及不良反应发生情况。结果 治疗3个月后研究组总有效率明显高于对照组(P<0.05)。治疗3个月后,两组患者白斑面积均明显小于治疗前,且研究组皮损区复色率明显高于对照组(P<0.05)。治疗3个月后,两组患者皮损区IL-1 β、ICAM-1水平均明显低于治疗前,且研究组低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 自体表皮移植术联合308 nm准分子激光治疗可更有效地缓解稳定期白癜风患者皮损区炎症反应,缩小白斑面积,增加皮损区复色率,提高临床治疗效果,安全有效,值得临床推广。Objective To investigate the therapeutic effffect of autologous epidermal transplantation combined with 308-nm excimer laser in the treatment of stable vitiligo.Methods Case data of 98 patients of stable vitiligo treated in our hospital from January 2019 to October 2022 were retrospective analysis,they were divided into a control group(40 cases)and research group(58 cases)based on treatment methods.The control group received autologous epidermal transplantation,while the research group received autologous epidermal transplantation combined with 308-nm excimer laser treatment.The clinical effifficacy,degree of pigment recovery in the lesion area,inflflammatory factors in the lesion area after 3 months of treatment and incidence of adverse reactions were compared between the two groups.Results The total effffective rate after 3 months of treatment of the research group was higher than that of the control group(P<0.05).The white spot area after 3 months of treatment of both groups were signifificantly smaller than before treatment,and the rate of skin lesion discoloration of the research group was signifificantly higher than the control group(P<0.05).The levels of IL-1β,ICAM-1 in the lesion area after 3 months of treatment of both groups were signifificantly lower than before treatment,and the research group was lower than the control group(P fiff<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The autologous epidermal transplantation combined with 308-nm excimer laser therapy can more fffleffectively alleviate the inflammatory response in the lesion area of stable vitiligo patients,reduce the area of white spots,increase the ffffcolor recovery rate of the lesion area,and improve clinical treatment effectiveness,it is safe and effective,and is worthy of clinical promotion.
