摘 要:目的探讨上下联合扩弓治疗对青少年骨性Ⅱ类错[牙合]患者气道体积的影响。方法选取40例青少年骨性Ⅱ类错[牙合]需要上下联合扩弓的患者作为研究对象,利用Mimics软件对上下颌扩弓前后的锥体束计算机断层扫描数据进行三维重建并分析气道变化。结果骨性Ⅱ类错[牙合]的患者经上下联合扩弓治疗后,其气道总体积与口咽段气道的体积均增大(t=2.362、2.890,P=0.006、0.004);口咽气道的最小截面积和L_(矢)/L_(横)值均增大(t=3.644、2.089,P=0.023、0.021)。鼻咽段与喉咽段体积也有所增大,但差异均无统计学意义(P均>0.05)。结论对处于生长发育高峰期的骨性Ⅱ类患者,上下联合扩弓治疗对其上气道体积,尤其是口咽段体积有显著影响,使上气道形态趋向卵圆形。Objective To evaluate the changes in the upper airway volume of patients with skeletal classⅡmalocclusion after maxillomandibular expansion treatment.Methods Forty patients with skeletal classⅡmalocclusion who needed maxillomandibular expansion were examined by CBCT before and after treatment.The Mimics software was used to reconstruct the CBCT data to analyze the changes of the airway.Results In patients with skeletal classⅡmalocclusion,the total upper airway volume and the oropharyngeal airway volume were significantly enlarged after maxillomandibular expansion(t=2.362,2.890,P=0.006,0.004).The minimum cross-sectional area of the oropharyngeal airway and sagittal and horizontal values were increased considerably(t=3.644,2.089,P=0.023,0.021).No significant changes were observed in the volumes of the nasopharyngeal and laryngopharyngeal airways(all P>0.05).Conclusion For adolescents with skeletal classⅡmalocclusion,maxillomandibular expansion can enlarge the upper airway volume,which can benefit respiration.
