摘　　要：目的分析肺结核分枝杆菌感染与趋化因子单核细胞趋化蛋白-1(MCP-1)、巨噬细胞移动抑制因子(MIF)和炎症因子白介素-6(IL-6)、白介素-1α(IL-1α)、白介素1β(IL-1β)水平的关系。方法选择2018年1月至2020年1月在河南科技大学第一附属医院就诊的肺结核分枝杆菌感染78例(结核组)及40例正常健康人群(对照组),采集受试者血清,采用酶联免疫吸附法与q-PCR法检测MCP-1、MIF和IL-6、IL-1α、IL-1β水平及其信使核糖核酸(mRNA)含量表达。结果结合组MCP-1、MIF、IL-6、IL-1α、IL-1β水平均高于对照组(P<0.05);结合组MCP-1、MIF、IL-6、IL-1α、IL-1βmRNA含量均高于对照组(P<0.05);血清MCP-1、MIF、IL-6、IL-1α、IL-1β与肺结核分枝杆菌感染均呈正相关(P<0.05);联合血清因子诊断敏感度、特异度高于单独MCP-1、MIF、IL-6、IL-1α、IL-1β(P<0.05)。结论MCP-1、MIF、IL-6、IL-1α、IL-1β参与肺结核分枝杆菌感染,且上述因子水平与疾病呈正相关,联合因子诊断可提高敏感度。Objective To analyze the relationship of Mycobacterium tuberculosis infection with the levels of the chemokines monocyte chemotactic protein-1(MCP-1),macrophage migration inhibitory factor(MIF),inflammatory factors interleukin-6(IL-6),interleukin-1α(IL-1α)and interleukin 1β(IL-1β).Methods Seventy-eight cases of Mycobacterium tuberculosis infection(TB group)and 40 healthy individuals(control group)in the First Affiliated Hospital of Henan Science and Technology University from January 2018 to January 2020 were selected;Serum and messenger ribonucleic acid(mRNA)levels of MCP-1,MIF,IL-6,IL-1αand IL-1βwere detected by enzyme-linked immunosorbent assay(ELISA)with q-PCR.Results The levels of MCP-1,MIF,IL-6,IL-1αand IL-1βin TB group were higher than those in control group(P<0.05).The mRNA contents of MCP-1,MIF,IL-6,IL-1αand IL-1βin TB group were higher than those in control group(P<0.05).Serum MCP-1,MIF,IL-6,IL-1α,and IL-1βlevels were positively correlated with Mycobacterium tuberculosis infection rate(P<0.05).The sensitivity and specificity of combined detection of serum MCP-1,MIF,IL-6,IL-1αand IL-1βwere higher than those of single detection of those indicators(P<0.05).Conclusion MCP-1,MIF,IL-6,IL-1αand IL-1βare involved in Mycobacterium tuberculosis infection,and the levels of these factors are positively correlated with disease progression.Combined detection of those indexes can effectively improve the diagnosis sensitivity.
