摘 要:耐药性结核病一直是传染病研究中的重难点之一,其在全球蔓延已严重威胁公共卫生,成为WHO消灭结核病策略的一大阻碍。新型抗结核药物贝达喹啉成为耐药性结核病患者的希望,提高了治愈率,降低了结核病复发率和死亡率,与传统抗结核药物相比具有降低耐药性的优势,但在我国临床使用时间相对较晚,其耐药相关基因研究较少,临床使用经验有限。本文结合相关文献,对结核分枝杆菌贝达喹啉耐药相关基因(rv0678、atpE、pepQ、rv1979c)及耐药机制研究进展进行综述,以期为我国结核病防治工作者提供借鉴与参考。Drug-resistant tuberculosis has always been one of the major difficulties in infectious disease research,and its global spread has posed a serious threat to public health,becoming a major obstacle to WHO's tuberculosis eradication strategy.The new anti-tuberculosis drug,bedaquiline,has become a hope for patients with drug-resistant tuberculosis,improving the cure rate and reducing the recurrence rate and mortality rate of tuberculosis.Compared with traditional anti-tuberculosis drugs,it has the advantage of reducing drug resistance.However,its clinical use in China is relatively late,the research on its resistance-related genes is lacking,and the clinical use experience is limited.Based on the relevant literature,this paper re⁃views the research progress on the genes related to bedaquiline resistance(rv0678,atpE,pepQ,rv1979c)and the mechanism of Mycobacterium tuberculosis,in order to provide reference for tuberculosis prevention and control workers in China.
