摘 要:目的以应力松弛、蠕变生物力学性能指标验证化学脱细胞同种异体神经联合骨髓间充质干细胞(bone mesenchymal stem cells,BMSCs)移植家兔坐骨神经缺损后的效果,为坐骨神经损伤移植选择合适的移植材料提供参考。方法首先建立家兔20 mm坐骨神经缺损模型,分别以自体神经、化学去细胞同种异体神经、化学脱细胞同种异体神经联合骨髓间充质干细胞移植;然后于移植24周时分别取出每组家兔实验侧坐骨神经各16个为实验组,随机取各实验组家兔右侧坐骨神经16个为正常对照组;最后在每组家兔中取1个坐骨神经标本进行组织形态观察,观察坐骨神经标本轴突、髓鞘、纤维排列情况,取15个坐骨神经标本进行应力松弛和蠕变实验,分析应力、应变与时间的变化规律。结果化学脱细胞同种异体神经移植组的7200 s应力下降量和7200 s应变上升量显著低于化学脱细胞同种异体神经联合骨髓间充质干细胞移植坐骨神经组和自体神经移植组(P<0.05);化学脱细胞同种异体神经联合骨髓间充质干细胞移植组坐骨神经的组织形态恢复效果好于化学去细胞同种异体神经移植组。结论化学脱细胞同种异体神经联合骨髓间充质干细胞具有恢复损伤神经的组织形态和应力松弛、蠕变弹性的作用,有利于损伤的坐骨神经的恢复和再生。Objective To verify the effect of chemically decellularized allogeneic nerve transplantation with bone mesenchymal stem cells(BMSCs)in a rabbit sciatic nerve defect model by stress relaxation and creep biomechanical performance indexes,and to provide reference for the selection of appropriate transplantation materials for sciatic nerve injury transplantation.Methods Firstly,a 20 mm sciatic nerve defect model was established in rabbits,and autologous nerves(ANT),chemically acellular nerve allograft(CANAT),and chemically acellular nerve allograft combined with BMMSCs(CANABT)were transplanted.Then,at 24 weeks of transplantation,16 sciatic nerves on each experimental side of each group of rabbits were taken out as the experimental group,and 16 sciatic nerves on the right side of each experimental group of rabbits were randomly taken as the normal control group.Finally,one sciatic nerve specimen was taken from each group of rabbits for histological observation,observing the arrangement of axons,myelin sheaths,and fibers of the sciatic nerve specimen;fifteen sciatic nerve specimens were taken for stress relaxation and creep experiments to analyze the changes in stress,strain,and time.Results The 7200 s stress decreases and the 7200 s strain increments in the CANAT group were remarkably lower than those in the CANABT and ANT groups,respectively(P<0.05).The morphological recovery effect of CANABT on the injured sciatic nerve was better than that of CANAT.Conclusions Chemically decellularized allogeneic nerves combined with bone marrow mesenchymal stem cells have the ability to restore the histomorphology and stress relaxation and creep elasticity of the injured nerve,which facilitates the recovery and regeneration of the injured sciatic nerve.
