摘 要:目的 探讨Carba NP试验与改良碳青霉烯类失活法(modified carbapenem inactivation method, mCIM)试验检测肠杆菌科细菌碳青霉烯酶的临床价值。方法 选取2021年3月—2022年6月如东县中医院临床分离的60株碳青霉烯类药物肠杆菌科细菌(carbapenem resistant enterobacteriaceae, CRE),分别对细菌进行Carba NP试验和mCIM试验检测,以碳青霉烯酶耐药基因聚合酶链反应(polymerase chain reaction, PCR)检测结果为金标准,计算两项试验的检测效能。结果 基于金标准,Carba NP试验检测肠杆菌科细菌产碳青霉烯酶的灵敏度与特异度分别为90.00%、93.33%,mCIM试验检测肠杆菌科细菌产碳青霉烯酶的灵敏度与特异度分别为93.33%、100.00%,两种检测方法诊断灵敏度与特异度比较,差异无统计学意义(P>0.05)。结论 Carba NP试验与mCIM试验均是检测肠杆菌科细菌碳青霉烯酶的有效可行方法。Objective To explore the clinical value of Carba NP test and modified carbapenem inactivation method(mCIM)test in the detection of enterobacteriaceae carbapenemase.Methods Sixty strains of carbapenem resistant en⁃terobacteriaceae(CRE)clinically isolated from the Rudong Hospital of Traditional Chinese Medicine from March 2021 to June 2022 were selected,and the bacteria were detected by Carba NP test and mCIM test respectively.The detec⁃tion results of carbapenemase resistance gene polymerase chain reaction(PCR)were used as the gold standard to cal⁃culate the detection efficiency of the two tests.Results Based on the gold standard,the sensitivity and specificity of Carba NP test for detecting carbapenase production by enterobacteriaceae bacteria were 90.00%and 93.33%,respec⁃tively,and the sensitivity and specificity of mCIM test for detecting carbapenase production by Enterobacteriaceae bac⁃teria were 93.33%and 100.00%,respectively.There was no significant difference in diagnostic sensitivity and speci⁃ficity between the two methods(P>0.05).Conclusion Both Carba NP test and mCIM test are effective and feasible methods for detecting carbapenemase of enterobacteriaceae bacteria.
