摘　　要：目的 通过回顾性分析咸宁地区男性精子DNA碎片指数(DNA fragmetation index of sperm, DFI)与患者年龄、精液参数的关系,为全面评估男性生育能力提供依据。方法 分析2021年8月—2022年8月在咸宁市中心医院生殖医学中心就诊的503例男性患者的精液常规及精子DNA碎片率,并将研究对象按DFI水平分为DFI≥25%组、DFI<10%组、10%≤DFI<25%组,分析各组受检者年龄、精液参数的变化;通过Spearman相关分析分析精子DNA碎片率与年龄、精液参数的相关性。结果 年龄越大,DFI指数越高,不动精子占比也越高,而精子总活力和正常精子形态率越低,差异有统计学意义(P<0.05)。进一步通过Spearman秩相关分析发现,精子DFI与患者年龄、不动精子呈正相关(r=0.214,0.359,P<0.001),与精子总活力、正常形态精子率呈负相关(r=-0.365,-0.137,P<0.001)。结论 通过年龄、常规精液检查以及精子DFI来综合评估男性的生育力将会更加全面。Objective To provide a theoretical basis for a comprehensive assessment of male fertility by retrospec⁃tively analyzing the relationship between sperm DNA fragmentation index(DFI)and patients'age and semen param⁃eters in men in the Xianning area.Methods The semen routine and sperm DNA fragmentation rate of 503 male pa⁃tients who visited the Reproductive Center of Xianning Central Hospital from August 2021 to August 2022 were ana⁃lyzed.The subjects were divided into DFI≥25%group,DFI<10%group,10%≤DFI<25%group according to DFI level.The changes in age and semen parameters of subjects in each group were analyzed.The correlation between sperm DNA fragmentation rate,age and semen parameters was analyzed by Spearman correlation analysis.Results The older the age,the higher the DFI index,the higher the proportion of motionless sperm,and the lower the total sperm motility and normal sperm morphology,the difference was statistically significant(P<0.05).Spearman rank cor⁃relation analysis showed that sperm DFI was positively correlated with age and immotile sperm(r=0.214,0.359,P<0.001),and negatively correlated with total sperm motility and normal sperm rate(r=-0.365,-0.137,P<0.001).Conclusion A comprehensive assessment of male fertility through age,routine semen examination and sperm DFI will be more comprehensive.
