摘 要:目的 观察大出血患者采用不同比例成分输血的效果。方法 选择2018年3月—2019年4月镇江市中医院应用大量输血的患者71例,根据输注血浆和去白细胞红细胞悬液比例分为3组,其中16例低比例组(新鲜冰冻血浆:红细胞<1∶3),37例中比例组(1∶3≤新鲜冰冻血浆:红细胞≤1:2),18例高比例组(1∶2<新鲜冰冻血浆:红细胞≤1∶1),对比3组患者入院后24 h凝血功能。结果 输血后,低比例组的凝血酶原时间(prothrombin time, PT)指标为(19.50±1.80)s、活化凝血酶原时间(activated prothrombin time, APTT)指标为(63.50±12.50)s、凝血酶时间(thrombin time, TT)指标为(25.70±6.50)s、纤维蛋白原(fibrinogen, FIB)指标为(1.10±1.30)g/L、血小板(platelet, PLT)指标为(82.60±12.10)×10^(9)/L。3组输血后PT、APTT、TT、FIB、PLT指标明显优于输血前,差异有统计学意义(P<0.05)。输血后,中比例组和高比例组的PT、APTT、TT均明显比低比例组短,差异有统计学意义(P<0.05)。结论 对急性大出血患者应用中高比例成分输血,有利于凝血功能恢复,加快患者止血。Objective To observe the effect of transfusion with different proportion components in patients with mas⁃sive hemorrhage.Methods From March 2018 to April 2019,seventy-one patients who received massive blood transfu⁃sion in Zhenjiang Hospital of Traditional Chinese Medicine were selected and divided into 3 groups according to the proportion of plasma transfusion and leucocyte removed red cell suspension.There were sixteen cases in the low pro⁃portion group(FFP:RBC<1∶3),thirty-seven cases in the medium proportion group(1∶3≤FFP:RBC≤1∶2),and eighteen cases in the high proportion group(1∶2<FFP:RBC≤1∶1).Blood coagulation function was compared 24 hours after ad⁃mission among the three groups.Results After blood transfusion,the PT index was(19.50±1.80)s,the APTT index was(63.50±12.50)s,the TT index was(25.70±6.50)s,the FIB index was(1.10±1.30)g/L,and the PLT index was(82.60±12.10)g/L.The indexes of PT,APTT,TT,FIB and PLT after transfusion were significantly better than those before transfusion,and the difference was statistically significant(P<0.05).After blood transfusion,the PT,APTT and TT in the medium proportion group and the high proportion group was significantly shorter in the medium low propor⁃tion group,and the difference was statistically significant(P<0.05).Conclusion The application of high proportion component blood transfusion in patients with acute massive hemorrhage is conducive to the recovery of coagulation function and the acceleration of hemostasis.
