妊娠期糖尿病伴亚临床甲减孕妇采用左旋甲状腺素对妊娠并发症的影响分析

摘　　要：目的探究左旋甲状腺素在妊娠期糖尿病伴亚临床甲减孕妇妊娠并发症中的应用效果。方法选取2022年3月—2023年3月潍坊市妇幼保健院收治的120例妊娠期糖尿病伴亚临床甲减孕妇为研究对象,随机分成常规组与左旋组,各60例。常规组采取常规治疗,左旋组在常规组的基础上采取左旋甲状腺素进行治疗。比较两组孕妇的血糖指标、甲状腺功能指标、并发症发生情况、不良妊娠结局情况以及生活质量。结果治疗后,左旋组的空腹血糖、餐后2 h血糖、促甲状腺激素以及游离甲状腺激素水平均低于常规组,生活质量评分均高于常规组,差异有统计学意义(P<0.05)。左旋组并发症发生率(6.67%)、不良妊娠总发生率(5.00%),低于常规组(20.00%、16.67%),差异有统计学意义(χ^(2)=4.615、4.227,P<0.05)。结论通过采取左旋甲状腺素进行治疗,能够有效改善孕妇的血糖水平以及甲状腺功能,降低并发症发生率,对不良妊娠结局进行改善,进而提高孕妇的生活质量。Objective To explore the application influence of levothyroxine on pregnancy complications of pregnant women with gestational diabetes mellitus and subclinical hypothyroidism.Methods A total of 120 pregnant women with gestational diabetes mellitus with subclinical hypothyroidism admitted to Weifang Maternal and Child Health Hospital from March 2022 to March 2023 were selected as the study objects and randomly divided into routine group and levo-rotation group,with 60 cases in each group.The conventional group received conventional treatment,and the levorotatory group received levothyroxine on the basis of the conventional group.Blood glucose,thyroid function,com⁃plications,adverse pregnancy outcomes and quality of life were compared between the two groups.Results After treat⁃ment,the levels of fasting blood glucose,2-hour postprandial blood glucose,thyrotropin and free thyroid hormone in levo-rotation group were lower than those in conventional group,and the quality of life scores was higher than those in conventional group,and the difference was statistically significant(P<0.05).The incidence of complications(6.67%)and the total incidence of adverse pregnancy(5.00%)in the levo-rotation group were lower than those in the conven⁃tional group(20.00%and 16.67%),and the difference was statistically significant(χ2=4.615,4.227,P<0.05).Conclu⁃sion Levothyroxine treatment can effectively improve the blood sugar level and thyroid function of pregnant women,re⁃duce the incidence of complications,improve adverse pregnancy outcomes,and thus improve the quality of life of preg⁃nant women.
