摘 要:目的分析干化学法与尿沉渣检验联合应用在尿常规检验中的作用。方法选取2020年8月—2022年8月南通大学附属启东医院检验收治的80例疑似泌尿系统疾病患者作为研究对象,采集晨起中段尿,均行干化学法与尿沉渣检验,以传统镜检结果为金标准,对比单独检测和联合检测方式诊断效果。结果金标准(传统镜检)显示:阳性52例,阴性28例。干化学法与尿沉渣检验联合应用诊断灵敏度为93.75%、特异度为93.75%、准确率为93.75%、阳性预测值为95.74%、阴性预测值为90.91%,明显高于单项检测方式,差异有统计学意义(P<0.05)。联合检测对红细胞、白细胞、结晶、管型和尿蛋白的总符合率明显高于干化学法,差异有统计学意义(P<0.05);诊断红细胞和管型的符合率均高于干化学法,差异有统计学意义(P<0.05);与干化学法比较,尿沉渣检验诊断有形成分的总符合率更高,差异有统计学意义(P<0.05)。结论干化学法与尿沉渣镜检联合应用可准确诊断尿液中的不同成分,也用于泌尿系统疾病诊断,有应用价值。Objective To analyze the role of dry chemical method and urine sediment test combined application in urine routine testing.Methods Select 2020 From August 2019 to August 2022,80 patients with suspected urinary system diseases admitted to Qidong Hospital Affiliated to Nantong University were selected as the research objects.The morning midstream urine was collected,and dry chemical method and urine sediment test were performed.The results of traditional microscopic examination were used as the gold standard,and the diagnostic effects of single detection and combined detection were compared.Results The gold standard(traditional microscopy)showed that 52 cases were positive and 28 cases were negative.The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined application of dry chemical method and urinary sediment test were 93.75%,93.75%,93.75%,95.74%and 90.91%,respectively,which were significantly higher than those of single detection method,and the differences were statistically significant(P<0.05).The total coincidence rate of combined detection of red blood cells,white blood cells,crystals,casts and urine protein was significantly higher than that of dry chemical method,and the difference was statistically significant(P<0.05);the coincidence rate of diagnosis of red blood cells and casts was higher than that of dry chemical method,and the difference was statistically significant(P<0.05);compared with dry chemical method,the total coincidence rate of urine sediment test in the diagnosis of visible components was higher,and the difference was statistically significant(P<0.05).Conclusion The combined application of dry chemical method and urine sediment microscopy can accurately diagnose different components in urine,which is also used for the diagnosis of urinary system diseases and has application value.
