微波消融术与传统开放手术在良性甲状腺结节治疗中对机体创伤影响的比较

摘　　要：目的对良性甲状腺结节患者使用微波消融术进行治疗的效果做出分析。方法随机选取2020年1月—2022年10月邵武市立医院收治的80例良性甲状腺结节患者作为研究对象,根据随机抽样法将患者分为两组,各40例,常规组使用传统开放手术治疗,研究组使用微波消融术治疗,对比分析两组患者的治疗效果,包括并发症、临床指标、炎性指标、甲状腺功能指标等。结果常规组并发症发生率20.00%高于研究组2.50%,差异有统计学意义(χ^(2)=6.134,P<0.05)。研究组手术时间、术中出血量以及住院时间均优于常规组,差异有统计学意义(P<0.05)。研究组治疗后的C反应蛋白、白细胞介素-6、肿瘤坏死因子-α水平均明显低于常规组,差异有统计学意义(P<0.05)。研究组治疗后的甲状腺激素、血清游离甲状腺素、血清游离三碘甲状腺原氨酸水平均高于常规组,差异有统计学意义(P<0.05)。结论为良性甲状腺结节患者使用微波消融术进行治疗,对患者甲状腺功能影响较小,不会增加患者炎性反应,患者并发症较少,对其机体创伤影响小,临床应用价值较高。Objective To analyze the effect of microwave ablation in patients with benign thyroid nodules.Methods 80 patients with benign thyroid nodules admitted to Shaowu Municipal Hospital from January 2020 to October 2022 were randomly selected as the research subjects.According to random sampling,the patients were divided into two groups,each with 40 cases.The conventional group was treated with traditional open surgery,while the research group was treated with microwave ablation.The treatment effects of the two groups of patients were compared and analyzed,including complications,clinical indicators,inflammatory indicators,thyroid function indicators,etc.Results The incidence of complications in the conventional group was 20.00%higher than that in the study group,which was 2.50%,the difference was statistically significant(χ^(2)=6.134,P<0.05).The study group had better surgical time,intraoperative bleeding volume,and hospital stay than the conventional group,the difference was statistically significant(P<0.05).C reactive protein,interleukin-6,and tumor necrosis factor in the study group after treatmentαThe levels were significantly lower than those of the conventional group,the difference was statistically significant(P<0.05).The levels of thyroid hormone,serum free thyroxine,and serum free triiodothyronine in the study group after treatment were higher than those in the conventional group,the difference was statistically significant(P<0.05).Conclusion The use of microwave ablation for the treatment of benign thyroid nodules has little impact on the patient′s thyroid function,does not increase the patient′s inflammatory response,has fewer complications,and has less impact on the patient′s body trauma.Its clinical application value is high.
