摘　　要：目的 探讨输尿管镜微创术治疗尿道损伤的临床效果。方法 将2020年1月-2022年10月在南京市高淳人民医院泌尿外科治疗的60例尿道损伤患者随机分为对照组和观察组,每组各30例。对照组使用开放性尿道吻合术,观察组使用输尿管镜微创术,对比两组的临床疗效、各项手术及恢复指标、并发症。结果 观察组治疗有效率为93.33%,明显高于对照组的70.00%(P<0.05);观察组手术时间、术中出血量、住院时间均少于对照组,MFR大于对照组(P<0.05),而膀胱冲洗时间、管道留置时间相当(P>0.05);观察组尿路感染、尿失禁、尿道狭窄、勃起障碍等并发症发生率明显低于对照组(P<0.05)。结论 输尿管镜微创术治疗尿道损伤的临床效果显著,创伤小,操作简便,术后恢复速度快,MFR恢复好,并发症发生率低。Objective To investigate clinical effect of minimally invasive ureteroscopy in treatment of urethral injury.Methods The paper divided 60 patients with urethral injury in urology department of Nanjing City Gaochun People's Hospital from January 2020 to October 2022 into control group and observation group randomly.Control group was treated with open urethral anastomosis and observation group with ureteroscopy minimally invasive surgery.Clinical effect,various surgical and recovery indicators and complications between two groups were compared.Results Effective rate of treatment in observation group was 93.33%,significantly higher than 70.00%in control group(P<0.05).Observation group had shorter surgical time,intraoperative bleeding volume and hospital stay than control group,with higher MFR than control group(P<0.05),while bladder flushing time and pipeline retention time was equivalent(P>0.05).Incidence of urinary tract infection,urinary incontinence,urethral stricture,erectile dysfunction and other complications in observation group were significantly lower than control group(P<0.05).Conclusion Ureteroscopic minimally invasive surgery can achieve significant clinical effect for urethral injury,with minimal trauma,simple operation,fast postoperative recovery,good MFR recovery and low incidence of complications.
