基于“肾主骨生髓”理论观察补肾活血汤对绝经后骨质疏松症β-catenin蛋白表达影响及相关性分析

摘　　要：目的:基于“肾主骨生髓”理论观察补肾活血汤(Bushen Huoxue Tang,BSHXT)对绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)患者β-catenin蛋白表达影响并进行相关性分析。方法:将符合纳入标准的72例PMOP患者随机分为观察组36例,对照组36例。对照组予以常规抗骨质疏松药物(阿仑膦酸钠和碳酸钙),观察组在对照组基础上予以补肾活血汤。在2组治疗前和治疗3个月后检测患者腰椎L_(1-4)BMD、左侧股骨颈BMD、骨转换指标(血清Ⅰ型原胶原N端前肽(PINP)、血清Ⅰ型胶原C-末端肽交联(S-CTX))、血清雌二醇(E_(2))、血清β-catenin蛋白水平、中医证候总积分及不良反应发生情况,并进行统计学分析。结果:治疗周期后,2组患者的腰椎L_(1-4)BMD、左侧股骨颈BMD、E_(2)、β-catenin蛋白水平均较治疗前增高,且观察组显著高于对照组;2组患者PINP、S-CTX、中医证候总积分均较治疗前降低,且观察组显著低于对照组;β-catenin蛋白与腰椎L_(1-4)BMD、左侧股骨颈BMD、血清E_(2)在治疗前后均呈正相关性,与PINP、S-CTX、中医证候总积分在治疗前后均呈负相关性,差异均有统计学意义。结论:补肾活血汤联合疗法可以有效增加PMOP患者骨密度,促进骨重塑正性平衡,提高患者体内E_(2)水平,改善患者中医证候,促进β-catenin蛋白表达,有效调控Wnt信号通路,能够有效干预PMOP患者的病情发展。Objective:To observe the effect of Bushen Huoxue Tang(BSHXT)on the expression ofβ-catenin protein in postmenopausal osteoporosis(postmenopausal osteoporosis(PMOP)patients)based on the theory of“kidney governing bone and marrow production”and to conduct correlation analysis.Methods:72 patients with PMOP who met the inclusion criteria were randomly divided into an observation group of 36 cases and a control group of 36 cases.The control group was given conventional anti-osteoporosis drugs(alendronate sodium and calcium carbonate),and the observation group was given Bushen Huoxue Decoction on the basis of the control group.Lumbar L1-4 BMD,left femoral neck BMD,bone turnover indicators(serum typeⅠprocollagen N-terminal propeptide(PINP),serum typeⅠcollagen C-terminal peptide)were detected before and after 3 months of treatment in the two groups.Cross-linking(S-CTX),serum estradiol(E2),serumβ-catenin protein level,total score of TCM syndrome and occurrence of adverse reactions were analyzed statistically.Results:After the treatment cycle,the levels of lumbar L1-4 BMD,left femoral neck BMD,E2,andβ-catenin protein levels in the two groups were higher than those before treatment,and the observation group was significantly higher than the control group;the PINP,S-CTX and total scores of TCM syndromes were all lower than before treatment,and the observation group was significantly lower than the control group;β-catenin protein was positively correlated with lumbar L1-4 BMD,left femoral neck BMD,and serum E2 before and after treatment,There was a negative correlation with PINP,S-CTX,and the total score of TCM syndromes before and after treatment,and the differences were statistically significant.Discussion:The combined therapy of Bushen Huoxue Decoction can effectively increase the bone mineral density of PMOP patients,promote the positive balance of bone remodeling,increase the E2 level in patients,improve the TCM syndrome of patients,promote the expression ofβ-catenin protein,and effectively regulate the Wnt s
