摘　　要：目的探讨曲美他嗪联合体外反搏治疗慢性心力衰竭的疗效及对转化生长因子-β_(1)(TGF-β_(1))、结缔组织生长因子(CTGF)的调节作用。方法选取2020年3月至2022年1月济南市第五人民医院收治的慢性心力衰竭患者90例作为研究对象,采用随机数字表法分为对照组与观察组各45例。两组均给予利尿、强心及扩张血管等常规治疗,对照组在其基础上增加体外反搏治疗,观察组在对照组基础上增加曲美他嗪治疗。比较两组临床疗效,治疗前后心率、收缩压、舒张压、心功能指标、血清TGF-β_(1)及CTGF水平,同时观察治疗前后的Lee氏心衰评分及明尼苏达州心功能不全生命质量量表(MLHFQ)评分。结果观察组治疗有效率明显高于对照组,治疗后与对照组相比左室舒张末期内径(LVDd)、左室收缩末期内径(LVDs)、左心房内径(LAD)降低,左室射血分数(LVEF)、左室流出道最大血流速度(VLOVT)较高,TGF-β_(1)、CTGF较低,Lee氏心衰评分及MLHFQ评分较低,差异有统计学意义(P<0.05)。结论曲美他嗪联合体外反搏治疗慢性心力衰竭,可有效改善患者的心功能,调节TGF-β_(1)及CTGF水平,确保提高患者的整体治疗效果以及生命质量。Objective To explore the efficacy of trimetazidine combined with extracorporeal counterpulsation in the treatment of chronic heart failure and the regulatory effect of transforming growth factor-β_(1)(TGF-β_(1))and connective tissue growth factor(CTGF).Methods A total of 90 patients with chronic heart failure admitted to Jinan Fifth People's Hospital from March 2020 to January 2022 were divided into control group and observation group with 45 cases in each group by random number table method.Both groups were given routine treatment such as diuresis,cardiac strengthening and vasodilation.The control group was additionally treated with external counterpulsation,and the observation group was additionally treated with trimetazidine.The clinical efficacy,heart rate,systolic blood pressure,diastolic blood pressure,cardiac function index,serum TGF-β_(1)and CTGF levels before and after treatment were compared between the two groups,and Lee's heart failure score and Minnesota Heart Dysfunction Quality of Life Scale(MLHFQ)score before and after treatment were observed.Results The effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the left ventricular end-diastolic diameter(LVDd),left ventricular end-systolic diameter(LVDs)and left atrial diameter(LAD)in the observation group were reduced compared with the control group,the left ventricular ejection fraction(LVEF),the maximum flow velocity of left ventricular outflow tract(VLOVT)were higher,and the TGF-β_(1)and CTGF were lower,Lee's heart failure score and MLHFQ score were lower,the difference was statistically significant(P<0.05).Conclusion Trimetazidine combined with external counterpulsation in the treatment of chronic heart failure can effectively improve the clinical symptoms and signs of patients,improve cardiac function indicators,regulate the levels of TGF-β_(1)and CTGF,and ensure the improvement of the overall treatment effect andqualityof life of patients.
